摘要
目的:观察氯诺昔康与丙泊酚联合应用于纤维肠镜检查时,对丙泊酚用量,苏醒时间及醒后肠痉挛痛影响。方法:100例纤维结肠检查患者,随机分成单纯丙泊酚组(P组)50例,氯诺昔康联合丙泊酚组(C组)50例。C组先静注氯诺昔康16mg,5min后静注丙泊酚1.5~2mg·kg^(-1),患者入睡开始肠镜检查,术中根据体动反应每次追加0.5mg·kg^(-1)丙泊酚,肠镜开始退出时停止用药。P组患者不用氯诺昔康,丙泊酚用法同C组。记录所有患者术前、术毕BP,HR和SPO_2,检查时间及苏醒时间(肠镜完全退出至患者呼之能应),丙泊酚用量,醒后肠痉挛痛采用VAS评分。结果:两组患者一般情况相似。两组患者术前术毕BP,HR,SPO_2均无统计学差异,但两组患者术毕BP、HR均低于术前水平。两组检查时间无统计学差异,C组唤醒时间明显短于P组,P组丙泊酚用量明显多于C组。C组有3例患者术后肠痉挛痛VAS评分大于4分,P组有4例。结论:氯诺昔康联合丙泊酚用于肠镜检查,能减少丙泊酚用量,缩短苏醒时间。
Objective: To evaluate the add-on effects of lornoxicam to propofol on propofol dosage,awakening onset and post-endoscopic analgesia on bowel spasm in patients undergoing the intestinal endoscopy. Methods: One hundred patients undergoing the intestinal endoscopy were randomly administered either propofol alone (n=50) or the combination of propofol with lornoxicam (n=50). The lornoxicam-added patients were i. v. injected with lornoxicam 16mg and subsequently administered with propofol (1.5 to 2mg·kg^(-1)) in 5 minutes. Following after the patients were asleep, the intestinal endoscopy was initiated. Propofol might be additionally administered at 0. 5mg·kg^(-1) every time based on the status of the body movement. No propofol was dosed once the endoscopy was completed. The patients in propofol group were administered with the same popofol regimen except lornoxicam doing as combination group. The BP,HR,SPO_2 were measured prior to and post the drug regimen. The awakening onset and the propofol dosage were assessed. The bowel spasm pain was scored with visual analgesia scale. Results:No significant differences of the BP,HR and SPO_2 between two groups were found. The awakening onset in the combination group was significantly earlier than propofol alone. The lornoxicam-added patients required less dosage of propofol compared with the patients with propofol alone. Three cases with bowel spasm pain were reported in the combination group but four cased in the propofol alone. Conclusion: Co-administration of lornoxicam with propofol reduces the propofol dosage and shortens the awakening onset in the intestinal endoscopy.
