支气管舒张试验阳性的COPD患者诱导痰炎性标志物检测的临床意义 被引量:5

摘要 目的:观察支气管舒张试验阳性的COPD患者诱导痰中嗜酸细胞(Eos)和嗜酸细胞阳离子蛋白(ECP)水平,探讨其诊断价值。方法:对86例COPD患者作肺功能检测,支气管舒张试验阳性患者为PCOPD组,阴性患者为NCOPD组,分别采用瑞氏染色及荧光免疫法检测高渗盐水诱导痰中细胞计数和ECP水平。选择2 0例支气管哮喘患者和10名健康人作为对照。结果:86例COPD患者中有11例支气管舒张试验阳性,阳性率为12 .8% ;PCOPD组患者诱导痰中Eos数量和ECP水平显著高于NCOPD组和健康组(P<0 .0 1) ;但显著低于支气管哮喘患者(P<0 .0 1)。结论:检测诱导痰Eos和ECP水平有助于鉴别支气管舒张试验阳性的COPD与支气管哮喘。 Objective:To investigate eosinophil and eosinophil cationic protein (ECP) levels in induced sputum of COPD patients with positive bronchodilation test to determine their value of differential diagnosis.Methods:Through lung function test 86 COPD patients were divided into PCOPD group with positive bronchodilation test and NCOPD group with negative bronchodilation test.The percentage of eosinophil and levels of ECP in induced sputum were measured by Wrights′ stain and Immuno-CAP System, respectively. The same index were measured in 20 patients with bronchial asthma and 10 healthy subjects as control.Results:12.8% (11/86) of the patients with COPD showed positive bronchodilation test. The percentage of eosinophil and levels of ECP of induced sputum in PCOPD group were significantly higher than those in NCOPD group and healthly group (P<0.01), but significantly less than those in bronchial asthma group (P<0.01).Conclusion:The detection of eosinophil and ECP in induced sputum should be helpful to the differential diagnosis between COPD with positive bronchodilation test and bronchial asthma.
