摘要
目的探讨中晚期前列腺癌去势术+姑息性TURP术后尿流动力学变化。方法对36例中晚期前列腺癌患者行去势术+姑息性TURP术,术后进行尿流动力学检查。结果36例患者均排尿通畅,无明显尿失禁者。3例有少许残余尿(10～25ml),余均残余尿。充盈期膀胱平均22.7cmH2O,平均最大尿流率21.3ml/s,PSA由术前平均37.63ng/ml下降至术后3个月的平均2.87ng/ml,9例伴有骨痛者术后明显减轻,国际前列腺症状评分(IPSS)明显降低,B超、IVP提示上尿路无积水。结论中晚期前列腺癌患者行去势术+姑息性TURP术后能获得满意的排尿,大大提高了病人的生活质量。
Objective To explore the assessment of urodynamics of mid-advanced stage prostate cancer treated by castroation and TURP.Method 36 patients of with mid-advanced stage prostate cancer treated by castration and TURP were reviewed,all of them were exmined by urodynamics.Results All the patients voided without difficulty,none of them suffered urinary incontinence.3 patients had small amount resideaal urine(10～25ml),the others had no.Maximal filling pressure was 22.7cmH 2O,Qmax 21.3ml/s,all the patients had no stagnant water of the upper urinary tract examined by 'B'ultrasonic wave and IVP.Mean PSA decreased from 37.63ng/ml preoperatively to 2.87ng/ml 3 months after the treatment.Bon pain in 9 cases was ameliorated.IPSS reduced significantly.Conclusions Castration and TURP is an effective method in treating mid-advancedstage prostate cancer.
