摘要
目的探讨颅鼻沟通瘤的诊断与手术治疗.方法回顾性分析31例颅鼻沟通瘤,肿瘤在颅内位于颅前凹,部分侵及颅中凹;25例出现鼻塞、鼻部外形改变,19例眼球突出、眼球移位、眼球活动障碍.采用显微外科技术行颅面联合入路切除肿瘤.结果手术全切28例,大部切除3例.均恢复良好,术后无死亡.随访7个月～9年,24例完全恢复健康,5例生活需要照顾,2例有不同程度恢复.结论颅眶沟通瘤多以鼻部、眼部和脑神经损害为主要临床表现.采用颅面联合入路,多科室合作,显微镜下切除肿瘤,可直接改善手术疗效.
Objective To study the diagnosis and microsurgical management of cranial nasal tumors. Methods 31 cases of cranial nasal tumors were retrospectively analyzed. Tumors were located in the anterior fossa and some extended into the middle fossa. 25 cases showed symptoms of nasal obstruction and deformation, and 19 cases manifested malfunction of optic organs. Microsurgical techniques as well as combined facial and intracranial approaches have been utilized to remove tumors. Results Tumors were totally removed in 28 cases, and subtotally in 3. All patients recovered well. No patient died during the follow-up period. Seven months to 9 years follow-up study showed that 24 cases resume normal life, 5 cases with dependent life, and 2 with recovery to some extent. Conculsion Most of the clinical manifestations of cranial nasal tumors represent disorders of nasal and optic organs. Microsurgical and piecemeal techniques as well as combined facial and intracranial approaches from different department may improve clinical results.
