摘要 目的 探讨用连续血液净化治疗感染性休克的疗效。方法 我院中心ICU自2 0 0 2年10月~2 0 0 4年10月间,用连续性静脉-静脉血液滤过(CVVH)治疗2 4例感染性休克患者,动态观察患者CVVH治疗前后血pH、氧合指数、HCO3 -、BE、BUN、SCr、血压、APACHE -Ⅱ评分的变化,并进行统计学分析,同时记录血管活性药的使用情况。结果 13例救治成功,11例死亡,病死率4 5 .8% ;治疗后APACHE -Ⅱ评分较治疗前明显降低,其它生化指标与治疗前比较有显著性差异;血管活性药用量可以很快减少。结论 对感染性休克患者及早进行连续血液净化,可避免发生不可逆性脏器功能损害,可提高救治成功率。 Objective To investigate the therapy effect of continuous blood purificatious for infection shock patients. Methods Twenty-four infectious shock patients received continuous veno-venous hemofiltration (VVH) therapy hospitalized in ICU department from oct. 2002 to Oct. 2004 were investigated. The patients' pH value, HCO - 3, BE, blood urea nitrogen (BUN), creatinine (Cr), blood pressure, APCHE-III scores, oxygenic index and dosages of asoactive drugs used before and after CVVH were observed. Results In the 24 patients, 13 survived and 11 died, the death rate was 45.8%. After receiving CVVH therapy, all patients' APCHE-II scores and biochemical paramenters were improved, dosage of vasoactive drugs were reduced.Conclusions For infectous shock patients, early continuous blood purification therapy may avoid multiple organ dysfunction syndrom (MODS) and improve porgnosis.
