新生儿寒冷损伤综合征的耳蜗电位和脑干听觉诱发电位变化

EFFECTS OF NEONATAL COLD INJURY SYNDROME ON COCHLEAR AND BRAINSTEM AUDITORY-EVOKED POTENTIALS

摘要 作者利用刺激延搁手段,对34例新生儿寒冷损伤综合征患儿进行了耳蜗电位和脑干听觉诱发电位的检测。结果发现,复温前异常率达92.86%,既有脑干功能障碍,又有蜗性听力障碍;复温后显著改善,但仍未恢复,异常率为50%;随防时异常率仍在30.77%,主要为外周型异常,而脑干功能已基本恢复。由此提示新生儿意外低体温为围产期听力损伤的高危因素之一,应列入听觉电生理筛查范围,对持续异常者应行进一步的听力学随访,以便早期诊断和干预。本文还就低体温影响脑干和听功能的部位及机理进行了探讨。 Cochlear receptor potential(CM and SP)and BAEPs were recorded simultaneously in 34neonates with cold injury syndrome by means of stimulation delay. The results showed that the incidence of abnormality was as high as 92.86%at the hypothermic stage,manifested by both dysfunctions of brainstem and peripheral auditory apparatus.Although improved markedly after being rewarmed,they were not reverted completely with an abnormality rate of 50%;follow up reported the abnormality rate remaining at 30. 77%. There was peripheral auditory dysfunction in the majority of them, while the brainstem function recovered basically. The study suggests that neonatal accidental hypothermia is one of the high-risk factors causing hearing loss in the perinatal period. Electrophysiologic screening tests should be made routinely in those infants before discharge. Further auditory follow up should be carried out in those with abnormal results so that auditory dysfunction may be detected and managed as early as possible. Analyses on the sites and the mechanism of the dysfunctions of brainstem and peripheral auditory apparatus in hypothermia were also discussed in this paper.
