摘要 目的研究局部亚低温治疗急性重症出血患者临床疗效及其对甲状腺激素的影响。方法随机将78例脑出血患者分为治疗组38例和对照组40例,治疗组在药物综合治疗基础上加用局部亚低温治疗,对照组为单纯药物综合治疗,观察两组治疗前、治疗后7、15d血浆甲状腺激素的变化及治疗前及治疗后30、90d临床神经功能评分及Barthel指数评分。结果局部亚低温组血浆FT3、FT4、TSH第7、15天逐渐恢复正常,明显优于对照组(P<0.05)。治疗30、90d后局部亚低温组临床ESS神经功能评分及Barthel指数评分明显高于对照组(P<0.05)。结论局部亚低温组治疗可改善脑出血患者甲状腺轴功能紊乱,并能促进患者神经功能恢复,明显改善预后。 Objective To study the effect of local mild hypothermia on the clinical significance of patients with acute severe cerebral hemorrhage and thyroid hormone.Methods 78 patients with cerebral hemorrhage were randomly divided into treatment group (38 cases) and control group (40 cases). Regular treatment was used in both groups, and the local mild hypothermia therapy was just applied in treatment group. The concentration of serum FT3,FT4, TSH in two groups before and the 7th, 15th day after treatment were compared. The scores of clinical neurological function and Barthel index before and at the 30th, 90th day after treatment were observed. Results The concentration of serum FT3, FT4, TSH in treatment group gradually returned to normal level at the 7th, 15th day after treatment, the outcomes were significantly better than those of the control group (P<0.05). The scores of clinical neurological function and Barthel index in treatment group were significantly higher than those in control group at the 30th, 90th day after treatment (P<0.05). Conclusions Local mild hypothermia therapy could obviously improve thyroid axis dysfunction, promote nerve function recovery and significantly improve prognosis of patients with cerebral hemorrhage.
