摘要
目的 探讨硫糖铝混悬液与山莨菪碱注射剂混合后鼻饲或口服 ,防治脑出血患者并发上消化道出血的疗效。方法 脑出血患者 1 4 9例分为三组。A组 56例鼻饲或口服硫糖铝混悬液与山莨菪碱混合剂 ;B组 51例静脉使用雷尼替丁。C组 42例为对照组 ,无任何预防上消化道出血措施。比较三组上消化道出血的发生率。结果 A组上消化道出血的发生率 1 .79%与B组 (1 3 .73 % )比较P <0 .0 5 ,与对照组C(30 .95 % )比较P <0 .0 0 5 ,差异有统计学意义 ,输血和病死率同步下降。结论 硫糖铝混悬液与山莨菪碱注射剂混合后鼻饲或口服对防治脑出血并发上消化道出血具有重要临床应用价值。
Objective To study the effect of mixture of Anisodamine and Ulcerlmin suspension by nasal feeding and oral administration against the upper digestive tract bleeding on patients with encephalorrhagia. Methods 149 patients with encephalorrhagia were divided into three groups. Group A, 56 cases were fed with anisodamine and Ulcerlmin suspension mixture within seven days. Group B, 51 cases were injected by ranitidine. Group C, 42 cases were controlled without any measures to prevent digestive tract bleeding without any measures to prevent digestive tract bleeding within seven days. The incidence of upper digestive tract bleeding in each group was recorded and analyzed. Results The incidences of the upper digestive tract bleeding were group A 1.79%,group B 13.73%(χ 2=3.909994,P<0.05) and group C 30.95%(χ 2=16.6736,P< 0.005 ).The incidence of bleeding in group A was distinctively lower than those in other two groups. The number of the patients requiring transfusion and mortality in group A declined simultaneously. Conclusion To prevent the stress induced upper digestive tract bleeding in patients with encephalorrhagia, it should not be limited to raising the PH value of the gastric juice. We should also be focused on improving the blood irrigation to the gastric mulosa, in order to ameliorate its acute pathological changes, which has a significant practical value in the prevention and treatment of the upper digestive bleeding in patients suffered encephalorrhagia.
