超低密度脂肪对比剂螺旋CT结肠成像初探 被引量:11

摘要 目的 探讨超低密度脂肪对比剂螺旋CT结肠成像的可行性。资料与方法  16例受检者肠道准备后 ,经直肠导管注入超低密度脂肪对比剂 (CT值 - 2 0 0~ - 2 4 0HU) 15 0 0ml,观察螺旋CT结肠成像质量 ,病变的检出率 ,受检者对超低密度脂肪对比剂的耐受性和对比剂的副作用。结果 结、直肠内充盈超低密度脂肪对比剂后 ,CT横断位图像质量良好 ,能顺利进行包括CT仿真内镜在内的螺旋CT结肠成像检查。结、直肠各段 1级扩张和准备分别为 71.1% (12 8个肠段中 91个 )和 4 8.4 % (12 8个肠段中 6 2个 )。 >10mm病变的检出率 88.9%。 14个结、直肠癌均检出且定位准确。受检者在检查期间和检查后 4 8h随访中无明显不适。 Objective To evaluate the feasibility of non-absorbable fat-dense contrast agent (CT value between -200 and -240 HU) for helical CT colonography. Materials and Methods After enema, 16 examiners were injected fat-dense contrast agent (CT value between -200 and -240 HU) 1 500ml through rectum catheter to evaluate the image quality of colonography, detection rate, tolerance and side effect of contrast media.Results Fat-dense contrast agent allows excellent visualization of the colorectal wall, mucosa and adjacent structures as well as forming good colonography. The percentage of colonic segments with grade 1 distention and preparation was 71.9%(91/128) and 48.4%(62/128) respectively with supine scanning. Detection rate of lesions larger than 10mm was 88.9%. All 14 colorectal carcinoma were detected and exactly located. Examiner had good acceptance and tolerance, without apparent side effects.Conclusion These preliminary results suggest that non-absorbable fat-dense contrast agent may be practical as a special new contrast medium for helical CT colonography to improving the detectability of lesions.
