老年CT肺密度测定与肺功能的相关性研究 被引量:1

摘要 目的初步探讨正常老年人群平均肺密度的特点及与肺功能的相关性。方法将肺功能正常者40例分为老年组25例(年龄60 ̄83岁,平均年龄72.5岁),中青年组15例(年龄27 ̄57岁,平均年龄48.65岁),进行CT平均肺密度及肺功能的测定。采用Siemens16层螺旋CT附带的Pulmo软件分别对全肺、主动脉弓、隆凸、肺静脉层面的平均肺密度进行测定。常规肺功能指标取FEV1的实测值与预测值的比值及FEV1/FVC、DLCO的实测值。并将两者行相关性分析,同时观察本组对象肺HRCT的细微结构的改变。结果测得的老年组全肺及上、中、下肺野的平均肺密度值分别为-732.29±48.35HU、-757.65±42.36HU、-773.78±46.12HU、-724.41±48.76HU。与中青年组比较仅中肺野密度有明显升高,且失去了自上而下的梯度改变,与常规肺功能指标无相关性。在肺HRCT上观察到13例下肺野背侧少许间质纤维增生,15例空气滞留。结论正常老年人群肺密度变化具有一定特点,但与常规肺功能指标无相关性。 Objective To study the correlation between MSCT and pulmonary function test(PFT) of lung density in the elderly. Methods In 40 volunteers, they were divided into the old group(n=25, 60 ̄83y, average 72.5±8.4y) and the younth age group (n=15, 27 ̄57y, average 48.65±7.6y). The CT scaning and PFT were respectively performed within one week for all cases. Both the lung density and the partial lung density on the aortic arch, carina, pulmonary vein slices were measured, by Pulmo analyzing software. The correlation between the lung CT density and the value of PFT (FEV1%, FEV1/FEV, DLCO) was performed. The subtlety structure of lung were observed on HRCT in the old group. Results The lung density value of total lung, upper lung, middle lung, lower lung were -732.29±48.35HU、-757.65±42.36HU、-773.78±46.12HU、-724.41 ±48.76HU respectively, only middle lung density was obviously arised, compared with middle age group, and the grades variety of lung density from up to down was vanished. There was no correlation between the lung density and the value of PFT. There were 13 cases with interstitial pulmonary fibrosis in bilateral lung inferior lobe by pleura, 15 cases with air trapping. Conclusions The variety of lung density was featured in the elderly group. Houseven, there are no correlation between the lung density and the value of PFT.
