摘要
目的探讨阴式子宫肌瘤剔出术的适应症,手术方法及预后。方法回顾性分析1998年4月～2001年8月经阴式子宫肌瘤剔出术40例患者的临床资料。结果 40例手术均经阴道顺利剔出肿瘤,没有1例转为开腹手术。术后病理回报均为子宫平滑肌瘤,术中出血量100～150ml,术后无感染及出血发生,术后三个月门诊复查无术后发病率。结论阴式子宫肌瘤剔出术,手术关键是作好适应症的选择,术前详细妇科检查及 B 超检查对于判断肌瘤大小,数目,部位及是否粘连非常重要。手术方式的选择不应千篇一律,应因人而异。阴式子宫肌瘤剔出术,手术创伤少,恢复快,但应具备熟练的阴式手术技巧。我们要求盆腔无粘连,肿瘤在8cm 以下开展阴式子宫肌瘤剔出术是完全适应的。
Objective To go futher into the indication,operation methods and the prognosis for the hystermoyomecomy through vagi- na.Methods By ways of retrospective analysis of clinical information about 40 cases of hysteromyomectomy through vagina performed during the period from April of 1998 to August of 2001.Results Out of the 40 cases of the hysteromyomectomy through vagina all the them showed good re- sults.The myomata were removed successfully.Moreover,none of case changes into the laparotomy.The postoperative pathology responds show as leiomyoma of uterus.The amount of bleeding in the course of operation is 100～150ml.After operation,there is no infection and bleeding.In addi- tion,within three months after operation,no postoperative incidence rate appears by ways of ambulatory reexamination.Conclusions For the hysteromyomectomy through vagina,the key lies in the good choices of indication.Prior to the operation,the detailed gynecologial exammination and ultrasonic B examination would be very important and necessary for the decisions of the size,number,position of the myomata as well as whether or not they are adhesive.The operation methods should not follow the same patterns of choice,whereas should make the appropriate chois- es according to the patient's conditions.The hysteromyomectomy through vagina gains the advantages such as the slight trauma and fast recovery, but it calls for the high operation skills.Generally,under the conditions of that there is no adhesion in the pelvic cavity and the size of myomata is less than 8cm,it is totally appropriate to perform the hysteromyomectomy through vagina.
