摘要
目的 报道1例以CD7+和双重t(8;21)易位为特征的近四倍体克隆急性髓细胞性白血病(AML)。 方法 本例AML患者骨髓细胞分别用R带技术进行染色体核型分析,流式细胞术进行免疫分型和细胞倍体分 析,逆转录聚合酶链反应(RT PCR)进行AML1 ETO融合基因检测。结果 染色体核型分析:46,xx,t(8;21)/80 81,xxx,-x,-1,-2,-3,t(3;5)(q29;q31),-4,-5,-7,t(8;21)×2,-9,-16,-17,-18,-19;免疫分型: CD7、CD13、CD33、HLA-DR、CD34阳性;细胞倍体显示双峰状(DI=2.05);RT PCR分析AML1 ETO融合基因阳 性;化疗后未获缓解,生存期4个月。结论 四倍体克隆与M2/t(8;21)白血病有密切的联系且为其继发性改 变,免疫表型表现为异质性,通常预后差,其可能为M2/t(8;21)的一个特殊亚型。
Objective To report a case of acute myeloid leukemia (AML) with CD7 + near-tetraploid clones by double t(8;21). Methods The chromosome was analysed by R-banding technique,immunophenotypic and DNA ploidy were analysed by flow cytometry,AML1-ETO fusion was transcripted by RT-PCR. Results Chromosome analysis showed a karyotype of 46,xx,t(8;21)/80,xxx,-x,-1,-2,-3,t(3;5)(q29;q31),-4,-5,-7,t(8;21)×2,-9,-16,-17,-18,-19.Immunophenotypic analysis were positive for CD7,CD13,CD33,HLA-DR,CD34.DNA ploidy showed double peaks representing diploid and near-tetraploid(DI=2.05).AML1-ETO fusion transcript was positive.The results of complete remission on chemotherapy could not be obtained and the patient survived only for 4 months. Conclusions Tetraploid clones are secondary events which are specifically associated with M2/t(8;21),the immunophenotypes reveal heterogenous and the prognosis is usually poor.It is possible that the tetraploidy clones with double t(8;21)is the special subtype of M2/t(8;21).
