肠道菌群失调原因菌的构成及耐药性分析 被引量:5

摘要 目的 了解医源性腹泻患者肠道菌群失调原因菌的构成及耐药性。 方法 用VITEK -ATB系统对该院2 0 0 1~ 2 0 0 3年间医源性腹泻患者大便经细菌培养和难辨梭状芽孢杆菌毒素A检测的肠道菌群失调原因菌进行统计分析。 结果 引起肠道菌群失调的原因菌按组成比大小依次是肠球菌 (3 1.90 % )、酵母样真菌 (2 7.2 4% )、变形杆菌(16.3 4% )、铜绿假单胞菌 (6.2 3 % )、枸橼酸杆菌 (5 .84% )、克雷伯菌 (5 .45 % )、产毒素A的难辨梭状芽孢杆菌 (5 .0 6% )、金黄色葡萄球菌 (1.94% )。其中酵母样真菌对两性霉素B和制菌霉素无耐药性 ,铜绿假单胞菌、变形杆菌和克雷伯菌对美洛培南和亚胺培南无耐药性 ,80 %的金黄色葡萄球菌为耐甲氧西林金黄色葡萄球菌 (MRSA) ,且对米诺环素、呋喃妥因、呋西地酸、万古霉素、替考拉宁和喹奴普汀 -达福普汀无耐药性。 结论 对医源性腹泻患者及时进行大便细菌培养及难辨梭状芽孢杆菌毒素A的检测能有效检测出引起肠道菌群失调的原因菌 ,从而有助于临床及时分析并去除引起腹泻的原因。 Objective To Understand the constitution and drug resistance of reason bacterium in intestinal tract dysbacteriosis of iatrogenic diarrhea patients. Method VTECK-ATB system was used to analyze statistically the monitoring data of reason bacterium from the intestinal tract dysbacteriosis by planting of bacterium and measuring toxin A of the clostridium difficle in stool of medical source diarrhea patient from year 2001 to 2003. Result Reason bacterium causing intestinal tract dysbacteriosis are ranked as following: enterococci (31.90%), yeast-like fungus(27.24%), proteus(16.34%), pseudomonas aeruginosa (6.23%), citrobacter (5.84%), klebsiella(5.45%), clostridium difficle to produce toxin A ( 5.06%), staphylococcus aureus (1.94%). The yeast-like fungus has no drug resistance to amphotericin B and nystatin. Pseudomonas aeruginosa, proteus and klebsiella have no drug resistance to imipenem and meropenem. Eighty percent staphylococcus aureus are MRSA, and have no drug resistance to minocycline,nitrofurantion,fusidic acid, vancomycin, teicoplanin and quinupristin-dalfopristin. Conclusion It is able to effectively examine reason bacterium from intestinal tract dysbacteriosis by planting of bacterium and measuring toxin A of the clostridium difficle in stool of iatrogenic diarrhea patient, thus it is helpful to make quick analysis and removal of reason bacterium who cause diarrhea in clinic.
