摘要 目的 :研究成人性激素 (E2 ,T)、甲状旁腺激素 (PTH)及骨钙素 (BGP)的增龄变化及性别差异 ,并探讨对骨代谢的临床意义 .方法 :正常成人 2 2 9名 (男 12 1,女 10 8) ,年龄 2 0~ 91岁 ,10岁分组 ;放射免疫法测定血清E2 ,T ,M -PTH ,BGP .结果 :女性E2 30~岁开始下降 ,4 0~岁开始显著下降 (P <0 . 0 1) ,5 0~岁下降 92 % ,>80岁下降 97% ;男性T 5 0~岁开始下降 ,6 0~岁开始显著下降 (P <0 . 0 5 ) ,>80岁下降 4 3% ;女性PTH 30~ 4 9岁低谷 ,5 0~岁显著升高 (P <0 . 0 1) ,并形成高峰 ,>80岁仍维持较高水平 ;男性PTH 5 0岁开始升高 ,>80岁仍高于青壮年 ,但差异不显著 (P >0 . 0 5 ) ;女性BGP 30~ 4 9岁低谷 ,5 0~岁显著升高 (P <0 . 0 1) ,6 0岁后逐渐降低 ;男性BGP 5 0~岁无升高 ,6 0岁后进行性降低 ,<80岁最低(P <0 . 0 5 ) .结论 :女性E2 下降早、多、快 ,男性T下降晚、少、缓 .绝经后PTH、BGP显著升高 ,符合绝经后骨代谢“高转换型” ,男、女老年期BGP下降符合老年骨代谢“低转换型” .继发PTH升高在绝经后骨质疏松症的发病中有重要作用 . Objective: To investigate the changes of adult sex hormones (E 2,T), parathyroid hormone (PTH) and bone gla protein (BGP) with aging and their clinical significance in bone metabolism.Methods: 229 healthy adults (121 male, 108 female) from 20 to 91 years old.Serum E 2, T, M-PTH and BGP were detected by radioimmunoassey. Results: The E 2 levels of female over 40 years old declined significantly (P<0.01), of 50~59 years old declined 92%, of >80 years old declined 97%.The levels of T in male over 60 years old declined significantly (P<0.05),70~79 and >80 years old declined 25% and 43% respectively.The PTH levels of female over 50 years old rose significantly (P<0.01), and the highest value in 50~59 years old.The PTH in male over 50 years old rose less than that of in female.The BGP level of female 50~59 years old rose significantly, and declined gradually after 60 years old, but that declined progressively in male with aging.Conclusion: The E 2 level of female with aging declines early, quickly and popularly, whereas the T level of male declines later, slowly and rarely.The PTH and BGP changes of postmaleopaul female accord with high-turnover type of bone metabolism. The BGP change of senile male accords with “low-turover type” of bone metabolism.The increase of secondary PTH plays an important rule on the pathogenesis of osteoporosis.
