摘要
报告１９９２年９月至次年１２月本校附属医院施行腹腔镜胆囊切除术（ＬＣ）３３０例．其中包括急性胆囊炎３５例和慢性萎缩性胆囊炎２１例；中转开腹手术１０例，开腹率３％，３３０例全部治愈，平均住院３．４天；发生重大并发症９例：胆总管切断２例，胆瘘３例，胆囊动脉出血３例，１２指肠痿１例，经处理全部治愈。讨论了ＬＣ的适应症与禁忌症，出血与胆囊动脉解剖，胆道损伤等问题，结合文献对ＬＣ进行了初步评价。
Presenting 330 cases undergoing laparoscopic cholecystec- tomy(LC)from Sept.1992 to Dec.1993 in our Affiliated Hosp.The patiants included 35 cases with acute cholecystitis and 21 cases with chro- nic atrophic cholecystitis.10 of the 330 cases accepted open cholecystecto- my during LC.All of the 330 were cured;the average time of hospitali- zation following surgery being 3.4 days.9 cases with severe complicati- ons,i.e.choledochus cutted 2 cases,cholascos 3 cases,bleeding of the cholecystic artery 3 cases,& duodenal fistula one case,all recovered follo- wing proper management.Problems of LC's indications & contraindicaticlis,bleeding during surg.,the anatomi- cal features of the cholecystic arte- ry,and the injury of biliary tract were discussed.Assooiating with the current literature & our own exprience,a preliminary evaluation of LC was made.
