摘要
采用一种简单、敏感的放射免疫法测定了101例肝病患者(25例肝癌,17例肝硬化,26例慢迁肝,33例乙肝病毒携带着)的血清循环免疫复合物,并观察了各组肝功能的变化以及HBV标志.结果表明:肝癌组的IgG-CIC含量及γ-GT含量明显高于其它各组.IgG-CIC 5.21(PHC),3.43(LC),2.41(CPH)及1.92(ASC),P<0.01.γ-GT 262(PHC).63.1(LC),44.7(CPH)以及18.7(ASC),P<0.01.伴有HBV感染的肝癌组中有80%可检出IgG-CIC,而CPH及ASC组中仅有50%可检出IgG-CIC.研究结果还表明:肝癌组中,IgG-CIC含量随着AFP含量的升高而上升.但在AFP过度升高时,IgG-GIC反而降低.这可能提示:随着肿瘤的生长,IgG-CIC升高,AFP值随肿瘤恶化逐渐上升,致使IgG-CIC从结合补体转变成非结合补体,导致IgG-CIC的下降.根据结果提示:HBV感染与肝癌发生有着密切关系.
The paper presented in to investigate the changes of circulating im-mune complexes (CIC) in various liver diseaes. CIC was measured with radioim-muoassay in 101 patients with liver diseases (including 25 hepatocellular carcioma (HCC), 17 liver cirrhosis (LC), 26 chronic persistent hepatitis (CPH) and 33 asymp-tomatic HBsAg carrier (ASC) alpha-fetoprotein (AFP), liver function and HBV marker were also measured at the same time. The results showed that IgG-CIC 5. 21 (HCC), 3. 43(LC), 2. 41 (CPH), 1. 92(ASC), P<0. 01 and γ-glutaminase 261. 6 (HCC), 63. l(LC), 44. 7(CPH), 18. 7(ASC), P<0. 001 were significatly higher in HCC patients than those in other groups. The positive rate of IgG-CIC was 84% in HCC, 76% in LC, 46% in'CPH and 54% in ASC. Among patients of HCC patients with HBV infection IgG-CIC positive rate is 80%, but only 50% positive in CPH and ASC, we also discovered that, as AFP elevated the IgG-CIC level increased signifi-cantly, but when AFP was higher than 40 000ng/ml, the IgG-CIC dereased reverse-ly. These results indicate that, 1. with the tumor growth, IgG-CIC increased, but further tumor burden might result in a fall of IgG-CIC; 2. as AFP incrased IgG-CIC may shift from complement-fixing to noncomplement-fixing; 3. there is close rela-tionship between HBV infection and HCC development.
