摘要
本文报告应用体外循环并深低温停循环为2例肾癌患者施行下腔静脉癌栓清除术。由于传统的外科技术在清除下腔静脉癌栓时受到限制,应用此种技术可充分暴露手术野,避免大血管损伤及失血所造成的损害。与常规体外循环并深低温停循环相比,如何保护好遗留的单侧肾脏并减少血液丢失是该技术用于此类非心脏手术的关键问题。
Embolectomy of cavai tumor emboli using cardiopulmonary bypass (CPB) with hypothermic circulatory arrest (HCA) was carried out in 2 patients with renal carcinoma. As traditional surgical procedure is limited for thorough removal of tumor emboli in the inferior vena cava, CPB with HCA had been employed. With this technique, the operation field was well exposed, the lesion was thoroughly cleaned, and massive bleeding from accidental injury of the great vessels could be avoided. Comparing with CPB and HCA in cardiac surgery, protection of the intact kidney and reduction of blood loss during and after CPB are special problems to be solved.
