急性颅脑损伤后脑脊液中β-内啡肽、强啡肽A1-13、心钠素和P物质的变化及其临床意义 被引量:17

摘要 对15例急性颅脑损伤患者脑脊液中β-EP、强啡肽Al-13、ANF和SP4种神经肽含量进行测定。结果发现,颅脑损伤后24h,β-EP、强啡肽Al-13和ANF含量较对照组均明显升高(P<0.01),重残和死亡患者的β-EP高于40ng/L、ANF高于1000ng/L;而SP含量较对照组明显降低(P<0.05)。伤后6~10天前3种物质含量降低,SP的降低更趋明显。提示:β-EP、强啡肽Al-13、ANF和SP可能参与了颅脑损伤后的继发病理过程。动态观察颅脑损伤患者脑脊液中这4种神经肽的含量变化,有助于探讨颅躺损伤急性期的病理演变过程,判断伤情及其预后。 The levels of β-endorphin(EP)、dynorphin A1-13、atrial natriuretic factors(ANF)and stbstance P(SP)were determined in the cerebrospinal fluid(CSF)of the 15 patients with acute brain injury.The re-ults showed that,24 hours after brain injury,the levelsof β-EP、dynorphin A1-131 and ANF weresignificantly higher than those of the control group(P<0.01),and β-EP levels were over 40 ng/L andANF over 1000 ng/L in the severely wounded pa-tients and the patients who died,but SP levels weresignificantly lower than those in the control group(P<0.05).6~10 days after injury,the levels of β-EP、dynorphin A1-13 and ANF dropped and SP droppedmore obviously.It is suggested that β-EP、dynorphinA1-13、ANF and SP are related to the secondary path-ological progress after brain injury.Dynamic observa-tion of the levels of the four nerve peptides in the CSFof patients with brain injury was helpful in investi-gating the pathological progress in the acute period af-ter brain injury and in evaluating the severity andprogosis.
