摘要
目的:分析不同类型脑缺血疾病与血浆纤维蛋白原的量和质及体外血栓模型各指标间的相关性。方法选取脑主干支梗死(MCI)患者26例,穿通支梗死(PCI)78例,一过性脑缺血发作(TIA)64例。正常对照56例,测定其纤维蛋白单体聚合反应速率(FM-PV)、纤维蛋白原浓度(Fg)、FMPV/ODmax互及体外血栓模型的长度、湿重、干重等指标。结果FMPV、FMPV/ODmax在各组间无区别(P>0.05),Fg在MCI组高于TIA和对照组(P<0.05);体外血栓湿重和干重在MCI和PCI组高于对照组(P<0.05);长度则无差异。多元回归分析发现FMPV/ODmax、体外血栓模型湿重等指标对脑缺血类型有明显影响。结论体外血栓模型湿重和干重增加是发生脑梗死的早期监测指标,高纤维蛋白原血症是MCI的病理基础之一,而纤维蛋白分子的聚集功能和体外血栓湿重是直接影响脑缺血发病类型的主要因素。
To explore correlativity of cerebral ischemia types with external thrombosis model to the fibrinogen molecular reactivity in plasma. Methods The fibrin monomer polymerization velocity(FMPV),FMPV/ODmax,fibrino ge n(Fg) were measured in 26 patients with cerebral arterial main trunks infarction (MCI ) , 78 patients with cerebral penetrating artery infarction,64 patients with TIA and 56 healthy subjects (CG). The length,moist and dry weight of external thrombosis model were also measured in them. Results FMPV,FMPV/ODmax were no significant difference among four groups (P>0.05). Fg was found to be raised in MCI groups as compared with TIA and control (P<0. 05 ).The moist are dry weight of external thrombosis model were found to be raised in MCl and PCI groups as compared with those of control (P<0. 05). The length was no significant difference among four groups (P>0. 05 ). FMPV /ODmax and dry weight of external thrombosis model were found to have notable influence on cerebral ischemia types by multi varlate regression analysis. Conclusions It is early monitoring pathogenic index of cerebral infarction that the moist and dry weight of external thrombosis model raise. Hyperfibrinemia is one of pathologic basls of MCI. Polymerization of fibrinogen molecular and dry weight of externa, thrombosis model are ma jor influence factors of cerebral ischernia types.
