摘要
目的探讨慢性胰腺炎(CP)的术式选择。方法回顾1987年到1996年手术治疗慢性胰腺炎40例。综合 CP 影像检查特征,以切除术治疗肿块型,胆胰管引流术治疗胆胰管扩张型作为基本的术式选择原则,并依据术中所见,对复杂型 CP 联合应用了奥狄括约肌成形,腹腔神经节切除,胆道取石,T 管引流等多种术式。结果术后27例腹痛消失,胰腺功能无明显恶化。2例分别死于急性胆管炎和上消化道大出血。结论影像检查可显示 CP 的主要病理变化。在影像检查的基础上,结合病人的主要症状,选择适当术式;或联合应用多种术式,可改善 CP 疗效。
Objective To evaluate the effect of surgical treatment on chronic pancreatitis(CP). Method From 1987 to 1996,forty patietns with chronic panereatitis were treated surgically.Accord- ing to the imaging features of B-US,CT,ERCP and operative pancreatogram,the patietns with pan- creatic mass were treated mainly with pancreatic resection,whereas those with dilated bile or/and pan- creatic duct were treated with bile or/and pancreatic duct drainage procedures.Sphincteroplasty,gan- gliectomy,lithotomy and T tube drainage were combined with the above-mentioned procedures accord- ing to intraoperative findings.Results Twenty-seven patients were free of pain postoperatively.There were no obvious deterioration of pancreatic function.Two patients died of acute cholangitis and massive hemorrhage.Conclusion This series of patients suggest that imaging techniques can show pathologic changes of CP.On the basis of imaging features and major symptoms,choice of appropriate procedure or combination of various modalities of operations will improve therapeutic efficacy in CP.
