摘要
目的 探讨抗凋亡蛋白Survivin在正常子宫内膜、不典型增生性子宫内膜及子宫内膜癌中的表达及其与Ki 67抗原表达的关系, 探讨Survivin蛋白在子宫内膜癌发生、发展中的作用及其作为判断预后因子的可行性。方法 应用免疫组织化学SP法, 检测10例正常子宫内膜、10例不典型增生性子宫内膜及 33 例子宫内膜腺癌中 Survivin蛋白及Ki 67抗原的表达, 并结合临床病理特点进行分析。结果 Survivin蛋白在不典型增生的子宫内膜和子宫内膜癌阳性表达率很高, 分别为87 9%和70 .0%, 二者差异无显著性意义 (P>0 .05), 但均高于在正常子宫内膜中的表达率10 .0% (P<0. 05); 在子宫内膜癌中, Survivin的表达与手术 病理分期和组织学分级密切相关 (P<0 .05), 但与肌层浸润无明显关系。另外, Survivin的表达还与 Ki- 67 的表达呈正相关 (P<0 .05)。结论 Survivin可能通过抑制凋亡和促进增殖两方面的作用参与了子宫内膜癌的发生、发展, 其过度表达提示预后不良, 可能成为子宫内膜癌的预后判断因子。
Objective To study the expression of apoptosis related protein Survivin in endometrial carcinoma and its relationship with expression of antigen Ki-67. Methods Immunohistochemical SP method was performed to detect the expression of Survivin protein and antigen Ki-67 in 10 cases of normal endometrium, 10 cases of endometrial atypical hyperplasia, and 33 cases of endometrial carcinoma, and examined the relationship of the expression with clinicopathological variables. Results In normal endometrium, only proliferative phase was weakly positive for Survivin, but in endometrial carcinoma and atypical hyperplasia, there was abundant Survivin immunoreactivity in the nucleus and/or cytoplasm of the endometrial carcinoma cells (87.9% and 70.0%),which was significantly higher than that in normal endometrium(P<0.05 respectively).But there was no significant difference between the expression or rate of Survivin in carcinoma endometrium and atypical hyperplasia endometrium.Moreover,Survivin expression in endometrial carcinoma was correlated with histological grade and surgical-pathological stage, but not with the invasion to myometrium.Expression of Ki-67 was positively correlated with expression of Survivin. Conclusion Survivin may participate in the onset and progression of endometrial carcinoma through inhibiting apoptosis and promoting proliferation.Survivin could be a new biomarkers in the diagnosis, treatment and prognostic evaluation of endometrial carcinoma.
