对超声检查隆乳术后包膜挛缩与假体状况的检诊价值 被引量:2

US evaluation for the relationship between breast implants and capsular contracture after augmentation mammoplasty

摘要 目的 应用超声检查探讨隆乳术后包膜挛缩与假体状况的关系。方法 应用高频超声检查隆乳术后严重包膜挛缩 2 2例。记录假体植入年限、临床表现及手术中的状况。结果 超声检查示严重包膜挛缩患者呈现典型的纤维包膜增厚 ,4 4个乳房假体有破裂 /渗漏影像的 33个 ,占 75 %。术中发现乳房假体破裂 /渗漏的 35个 ,发生率为 79 5 % ,均为置入 5年以上的假体。超声检查诊断符合率为 77 1%。结论 隆乳术后 5年以上的乳房假体破裂 /渗漏是导致严重包膜挛缩的主要原因。超声检查有助于包膜挛缩的评估、假体破裂 /渗漏的诊断和手术治疗的选择。 Objective To assess the value of US in evaluating the relationship of capsular contracture after augmentation mammoplasty and implant integrity. Methods The ultrasound examination was referred to 22 cases for severe capsular contracture after augmentation mammoplasty. The clinical condition, the years of implantation and surgical explantation were also recorded in 28 capsular contracture patients. Results A total of 56 implants were removed and rupture/leakage was found in 38 implants(67.9%). In 33 rupture/leakage implants reported by US(75%) and 35 rupture/leakage implants during surgical explantation(79.5%) for 44 implants in 22 patients, the US-surgical positive rate was 77.1%. All were implanted over 5 years. Conclusion The most of breast implants may be ruptured or leaked over 5 years and it is the major factor resulting in the severe capsular contracture after augmentation mammoplasty. US is considered as an effective and reliable tool for breast implant integrity and clinical evaluation of capsular contracture.
