p16 and cyclin D1 gene expressions in gallbladder carcinoma tissue

摘要 目的 :探讨细胞周期调控因子 p16和CyclinD1蛋白表达在胆囊癌发生发展中的作用及其对预后影响 .方法 :采用S P免疫组化法对 4 1例胆囊癌、7例胆囊腺瘤、14例胆囊炎组织中 p16 ,CyclinD1进行了检测 .结果 :胆囊癌组织中p16阳性率为明显低于胆囊炎组 (4 8.8%vs 92 .9% ,P <0 .0 1) ;胆囊癌 (6 8.3% )和腺瘤组 (5 7.1% )CyclinD1表达明显高于胆囊炎组 (6 8.3%vs 7.1% ,P <0 .0 5 ) .随着肿瘤恶性程度的增高和临床分期的进展 ,p16阳性率下降 ,CyclinD1阳性率有升高趋势 .p16在存活组的表达率显著高于死亡组(75 .5 %vs37.9% ,P <0 .0 1) .CyclinD1在存活组的表达率显著低于死亡组 (4 1.7%vs 79.7% ,P <0 .0 5 ) .p16与Cy clinD1的表达存在负相关性 .有 4 1.5 %的病例存在 p16和CyclinD1同时表达或失表达 .结论 :p16与CyclinD1异常表达在胆囊癌的发生。 AIM: To investigate p 16 and cyclin D1 gene expressions and to evaluate their significance in carcinogenesis, development and prognosis of the gallbladder carcinoma. METHODS: p 16 and cyclin D1 gene expression in 41 cases of gallbladder carcinoma (GBC), 7 cases of gallbladder papilloma and 14 cases cholecystitis were detected by immunohistochemistry. RESULTS: Positive rate of p 16 gene expression in GBC (48.8%) was lower than that in cholecystitis (48.8% vs 92.9%, P <0.05) while the positive rate of cyclin D1 gene expression in GBC (68.3%) and gallbladder papilloma was higher than that in cholecystitis (68.3% vs 7.1%, P <0.05). With the progress of the tumor grade and clinical stage, the positive rate of p 16 gene expression in GBC decreased while that of cyclin D1 increased. p 16 gene expression in the non recurrent group and the survival group was higher as compared with that in the dead group (75.5% vs 37.9%, P <0.05). While cyclin D1 in the survival group was lower as compared with that in the dead group (41.7% vs 79.7%, P <0.05), p 16 gene expression was negatively correlated to the cyclin D1 gene expression. Absence and presence of both the genes occurred in 41.5% of all the cases. CONCLUSION: Abnormal expression of p 16 or cyclin D1 genes plays an important role in the genesis and progression of GBC.
