肢体移植大鼠应用FK506对细胞凋亡及其预后的影响

摘要 目的:采用大鼠异体肢体移植模型观察同种异体肢体移植急性排斥反应中的细胞凋亡,并探讨FK506对移植肢体细胞凋亡及预后的影响。方法:实验于2003-11/2004-05在哈尔滨医科大学附属第一医院实验动物中心实验室完成。供体为Wistar大鼠,受体为SD大鼠,行大鼠异体肢体移植,对照组不给予任何免疫抑制剂,实验组注射FK5061mg/(kg·d)。苏木精-伊红(HE)染色进行移植组织排斥反应的病理分级,原位末端标记(TUNEL)法检测移植组织细胞的凋亡情况,计算凋亡指数。结果:对照组移植肢体术后(3.43±0.79)d开始出现皮下水肿、皮色变红,平均存活时间为(7.42±1.72)d,实验组未见明显排斥反应。两组间皮肤组织排斥反应病理分级术后3,5,7d对照组分别为(1.14±0.38),(2.28±0.48)和(2.86±0.38)级、实验组为(0.42±0.53),(0.57±0.53)和(0.86±0.38)级(P<0.01)。发生凋亡的细胞主要是皮下组织内浸润的淋巴细胞,两组间凋亡指数术后3d对照组为(8.85±2.58)个、实验组为(4.60±2.42)个(P<0.05),术后5,7d对照组为(13.63±3.12)和(17.59±4.91)个、实验组为(5.52±2.46)和(7.35±2.38)个(P<0.01)。结论:细胞凋亡是同种异体肢体移植急性排斥反应中组织损伤的重要机制,FK506能明显减少移植肢体细胞凋亡的发生,延长移植肢体存活时间,FK506? AIM:To investigate cell apoptosis in acute rejection of limb allograft in rats models, and explore the influence of FK506 on the apoptosis of allografted limb and prognosis of rats. METHODS:The experiment was done in the Experimental Animal Center,First Affiliated Hospital of Harbin Medical Unviersity from November 2003 to May 2004.The Wistar rats were taken as the donors,while SD rats as receptors.The SD rats underwent limb allotransplantation from allogenetic Wistar.Rats in the control group were not given any immunosuppressive agents,while those in the experimental groups were injected with FK506 1 mg/kg per day.Histopathological rejection grading in the grafted tissue was performed with hematoxylin eosin(HE) staining.Apoptosis cells were detected by terminal deoxynucleotidyl transferase mediated deoxyuridine triphosphate nick end labeling(TUNEL),and the apoptosis index(AI) was calculated. RESULTS:The grafted limb of the control group had edema and erythema at(3.43± 0.79) days after transplantation. The average survival time was(7.42± 1.72) days,while there was no obvious signs of rejection in the experimental group.The histopathological rejection grading of skins on the 3rd, 5th and 7th days after operation in the control group were(1.14± 0.38),(2.28± 0.48) and(2.86± 0.38) grades, and those in the experimental group were(0.42± 0.53),(0.57± 0.53) and(0.86± 0.38) grades(P< 0.01).The apoptotic cells in allografts were mainly infiltrating lymphocytes in epidermal cells.AI on the 3rd day in the control group and experimental group were 8.85± 2.58 and 4.60± 2.42(P< 0.05), and those on the 5th and 7th days after operation were 13.63± 3.12 and 17.59± 4.91 in the control group,but 5.52± 2.46 and 7.35± 2.38 in the experimental group(P< 0.01). CONCLUSION:Apoptosis is an important mechanism of tissue injury in acute rejection of limb allograft,and FK506 can apparently reduce the apoptotic cells in allografts and prolong the survival time of the grafted limbs.The protective effect of FK506 on the allogr
