摘要
目的:探讨前路减压植骨、钢板内固定治疗陈旧性胸腰段脊柱骨折截瘫患者神经功能恢复的作用。方法:中南大学湘雅医院1999-08/2003-07采用前路椎体切除减压、植骨融合、钢板内固定术对19例陈旧性胸腰段脊柱骨折并截瘫患者进行治疗,术前术后均对患者神经功能进行Frankel分级评估。结果:术后随访5.3~27个月(平均16.4个月),3~6个月(平均3.9个月)所有病例植骨融合,术后后凸Cobb角由术前平均17.5°恢复到6.6°,内固定无松动及断裂,瘫痪均得到不同程度恢复。结论:前路手术减压彻底,采用钢板内固定稳定性可靠,植骨融合快,能有效促进陈旧性胸腰椎骨折截瘫患者脊髓功能的恢复。
AIM:To explore the efficacy of anterior decompression, fusion and plate fixation on the recovery of neurological function in patients with paraplegic patients with old thoracolumbar vertebrae fractures. METHODS:Nineteen patients with old thoracolumbar fractures accompanied by paraplegia were treated with anterior decompression,fusion and internal fixation with anterior plate fixation systems in the Xiangya Hospital of Central South University from August 1999 to July 2003.Preoperative and postoperative neurological functions were assessed by using Frankel grading. RESULTS:All the patients were followed up for 5.3 to 27 months(16.4 months on average) after operation,the fusion time was 3 to 6 months(3.9 months on average),and postconvexity of Cobb's angle recovery from 17.5° before operation to 6.6° after operation,besides,no plate and screw was loose and fractured,and paraplegia were recovered to different degrees. CONCLUSION:Anterior approach provides thorough decompression,anterior plate fixation systems have reliable stability,and fusion is fast,which can effectively promote the recovery of spinal function of paraplegic patients with old thoracolumbar fractures.
