摘要  目的 探讨上海社区自然人群不同年龄组中基础胰岛素水平的变化。方法 将 2 120例正常糖耐量者以每 10岁为单位分为 7个亚组。所有研究对象均测简易体脂参数、血压、空腹血糖、胰岛素及血脂谱,以稳态模式评估法的胰岛素抵抗指数 (HOMA IR)估测机体胰岛素抵抗状况。结果 ①体重指数 (BMI)、腰臀比(WHR)、三酰甘油(TG)、总胆固醇(TC)、低密度脂蛋白(LDL)、收缩压(SBP)、舒张压 (DBP)均随年龄增加而增高,而高密度脂蛋白(HDL)随年龄增加而降低。50岁以前,随着年龄增长空腹胰岛素(FIN)水平逐渐下降,而在50岁以后这种变化消失。校正性别、BMI、TG、空腹血糖 (FPG)和HOMA IR后,各年龄组中FIN与未校正前相似。②直线回归分析显示, 50岁以前年龄增长与FIN成负相关,BMI、TG与FIN成正相关; 50岁以后BMI、TG与FIN仍成正相关。多因素分析显示,年龄及BMI对HOMA IR的影响尤为突出。结论 ①年龄增长与基础胰岛素水平呈非线性相关, 50岁以前年龄增长与FIN成负相关, 50岁以后年龄增长对基础胰岛素影响消失。②BMI、TG水平对FIN水平呈正性影响作用。 Objective To investigate the changes of basic insulin level in different age-group in Chinese.Methods 2 120 normal glucose tolerance subjects (age 15-94 yrs, 832 men, 1 288 women) were divided into 7 age-subgroups: age<20 yrs, 20-29 yrs, 30-39 yrs, 40-49 yrs, 50-59 yrs, 60-69 yrs and≥70 yrs. Fasting serum insulin(FINS), triglyceride(TG), fasting plasma glucose(FPG) and body mass index(BMI) were determined in all subjects. Insulin resistance was evaluated by the insulin resistance index of hemostasis model (HOMA-IR).Results ① With increase of age, FIN levels were 18.65±0. 54,17.39±0.57, 16.39±0.35, 13.55±0.27, 13.78±0.41, 14.32±0.34 and 13.46±0.40 mU/L in the 7 age-subgroups respectively, which showed that fasting insulin levels proportionately was reduced with aging in those subjects under 50 yrs, but no significant changes in the subjects≥50 yrs. Meanwhile, BMI and TG showed the trends to increase with aging. Adjusting the sex, BMI, TG and FPG; the FINS concentration in each age-subgroup was similar to those of the unadjusted data. ② Results of multiple linear regression analysis revealed that: 1) among those subjects<50 yrs, FINS was negatively correlated with aging, with age-increase of one year, FINS decreased 0.2 mU/L. However there was a positive effect of BMI and TG on FINS, when BMI increased 1 kg/m2 or TG increased 1 mmol/L; FINS also increased 0.54 mU/L or 1.02 mU/L respectively. 2) Among those subjects aged≥50 yrs, the major factors affecting basic insulin level were BMI and TG. When BMI increased 1 kg/m2 or TG increased 1 mmol/L, FINS increased 0.62 mU/L or 0.82 mU/L respectively. Conclusions ①The relationship between aging and basic insulin level is non-linear, fasting insulin level decreases with aging in subjects<50 yrs and the influence of aging disappears in subjects≥50 yrs. ②There are positive effects of BMI and TG on basic insulin level.
