外源性糖皮质激素对危重病人早期甲状腺功能的影响 被引量:2

Effect of Exogenous Glucocorticoids on Thyroid Function in the Early Stage of Critical Patients

摘要 目的观察外源性糖皮质激素对危重病人早期下丘脑-垂体-甲状腺轴的影响。方法对59例使用外源性糖皮质激素的危重病人早期进行甲状腺激素和促甲状腺激素测定,并与同期未用外源性糖皮质激素的危重病人进行比较。结果使用甲强龙组及地塞米松组 TT3均显著低于未用激素组(P<0.05或 P<0.01);甲强龙组 TT4、FT3、TSH 与未用激素组比较差异无显著性;地塞米松组 TT4、FT3、TSH 均显著低于未使用激素组(P<0.05或 P<0.01);甲强龙组及地塞米松组 FT4、rT3与未用激素组比较差异无显著性。结论外源性糖皮质激素能够影响危重病人早期下丘脑-垂体-甲状腺轴的功能,且不同种类的外源性糖皮质激素对其影响程度也不完全相同。 Objective To observe the effect of exogenous glucocorticoids on hypothalamic-hypophysial-thyroid axis in the early stage of critical patients.Methods The serum thyroid hormones and thyrotropin were measured in the early stage of 59 critical patients and the critical patients of non using exogenous glucocorticoids.Results The serum level of TT3 in methylprednisolone group and dexamethasone group were significantsly lower than of not using exogenous glueocortieoids group(P<0.05 or P<0.01).No differences in serum TT4、 FT3、TSH levels were found between methylprednisolone group and of not using exogenous glucocorticoids group.The serum levels of TT4、 FT3、TSH in dexamethasone group were significantly lower than of not using exogenous glucocorticoids group (P<0.05 or P<0.01).No differences in serum FT4、rT3 levels were found between methylprednisolone group or dexamethasone group and of not using exogenous glu- cocorticoids group.Conclusion Exogenous glucocorticoids could effect hypothalamic-hypophysial-thyroid axis of the early stage of critical patients and different exogenous glucocorticoids have different effect.
