无代谢异常的机关背景中老年人糖代谢状态分析

摘要 目的 观察常规体检无代谢异常的机关背景中老年人群糖耐量低减患病情况。方法  10 0 3 9名机关背景人群行常规体检 ,对筛查出的10 8名无代谢异常的中老年人群进一步行 75 g口服葡萄糖耐量试验 (OGTT)。 结果  10 8名受试者经 75 gOGTT检查 ,共检出DM 1人 (0 .93 % ) ,IGT 16人 (14 .81% ) ;IGT组年龄、收缩压、空腹血糖、OGTT 2h血糖、OGTT 2h胰岛素、甘油三酯及Homa IR均明显高于正常糖耐量(NGT)组 (P值分别 <0 .0 1,0 .0 5 ) ;logistic回归分析显示 ,本组研究人群发生IGT的危险因素为年龄、空腹血糖和甘油三酯水平 (P值均 <0 .0 1)。结论 常规体检无其他代谢异常的机关背景中老年人仍属IGT的高发人群 ,存在相对的胰岛素抵抗、高血压、高甘油三酯等病理生理状态 ,应积极开展IGT的早期筛查 ,早期干预。 Objective To investigate the impaired glucose tolerance (IGT) state of the middle-aged and the elderly with office backgrounds among whom no metabolic abnormalities were found in routine physical examination.Methods 108 middle-aged and old persons whose metabolisms were normal were selected from 10,039 ones with office backgrounds who experienced routine physical examination, and then were tested with oral glucose tolerance test (OGTT). Results Out of the 108 who underwent OGTT, one (0.93%) was diagnosed with type 1 diabetes mellitus (DM1), and 16 (14.81%) of IGT. In those who suffered from IGT, elder age, higher systolic pressure and higher levels of empty stomach blood sugar, 2~h 75~g OGTT blood sugar, triglyceride (TG), and higher homeostasis model assessment of insulin resistance (Homa-IR) index were found (P<0.01 or P<0.05). Logistic regression analysis showed that the dangerous factors that led to IGT were age and the levels of empty stomach blood sugar and TG (P<0.01). Conclusion The middle-aged and the elderly with office backgrounds, among whom no metabolic abnormalities were found in the routine physical examination, are still the highly dangerous group that may suffer from IGT. There are relative insulin resistance, hypertension and high blood TG level among them. So early IGT screening and intervention should be performed.
