摘要
本文对10例正常人(Ⅰ组),13例 COPD 患者(Ⅱ组)和17例呼衰气管插管患者(Ⅲ组)进行了最大口腔吸气压(PImax)和最大经膈压(Pdimax)的测定,三组 Pdimax 依次为11.29±3.02kPa、8.11±2.d6kPa 和6.22±2.35kPa、PImax 依次为8.63±2.80kPa、6.62±2.11kPa 和5.76±2.32kPa。各组 Pdimax 均大于该组的 PImax、Pdimax 测定值以第Ⅲ组最低,Ⅱ组次之,Ⅰ组最高,三组PImax 除Ⅰ组与Ⅱ组有明显差别外,Ⅲ组与Ⅱ组间及Ⅱ组与Ⅰ组间 PImax 的差异尚未达到统计学意义,提示 Pdimax 较 PImax 更早地反应呼吸肌(尤其是膈肌)功能损害,尽管如此,三组 PImax 与Pdimax 均呈直线相关,其相关系数分别为0.78、0.86和0.94,证明 PImax 作为无创伤性呼吸肌力量评价指标,仍有一定实用价值。
Maximal inspiratory pressure(PImax)and maximal transdiaphragmatic pressure(Pdimax)were measured in 10 normal subjects(group Ⅰ),13 stable COPD patients(group Ⅱ)and 17 intubated COPD patients(group Ⅲ).Pdimax in the three groupswere 11.29±3.02 kPa,8.11±2.46 kPa and 6.22±2.35 kPa,respectively.PImax were8.63±2.80 kPa,6.62±2.11 kPa and 5.76±2.32 kPa,respectively.The results showedthat PImax was significantly lower than Pdimax in all the three groups.Group Ⅰ hadthe highest Pdimax,group Ⅱ had the intermediate Pdimax,while group Ⅲ had thelowest Pdimax.Except that PImax of group Ⅲ was lower than that of group Ⅰ,therewere no significant differences of PImax either between group Ⅲ and group Ⅱ or between group Ⅱ and group Ⅰ.It was suggested that Pdimax could detect the impairmentof the function of respiratory muscles earlier than PImax.As PImax and Pdimax keptgood linear relationship in all the three groups(r values in three groups were 0.78,0.86 and 0.94,respectively),PImax still had its practical value as a non-invasiveand convenient test to evaluate the strength of respiratory muscles.
