摘要
30例腰椎间盘突出症作椎间盘部分切除术。切除的椎问盘称重,并对手术前后的X线和CT图象进行测量。手术后椎间盘高度的丧失与切除的椎问盘重量呈线性关系。平均每切除lg椎间盘,其高度丧失8.76%,同时纤维环膨出有增加。认为椎间盘切除术使椎间盘内压骤降,纤维环失去原有张力,导致椎间盘高度丧失和纤维环膨出增加。这可能是腰椎不稳定和神经根受损的原因。
Partial discectomy was performed on 30 patients with protrusion of lumbar intervertebral disc. The disc tissue resected was weighed and the x-ray and CT films taken before and after operation were scrutinized. The reduction of the disc height was almost directly proportional to the weight of the disc tissue excised, a mean reduction of 8.76% of the original height (per gram of disc tissue excised). It was considered that lumbar discectomy reduced the intradiscal pressure suddenly, the annulus fibrosus lost its normal tensile strength, and the movement of the involved segments becalm slackened. In such a way, the disc height decreased, while the disc bugle increased. And these may be the underlying causes of instability of the lumbar vertebrae and nerve root damage.
