摘要
目的 对回盲肠间置幽门替代术应用于胃肠道重建中作初步研究。方法 将 2 1例成年健康小型猪分为 3组 :假手术对照组、B I式组和远端胃大部切除带蒂回盲肠间置幽门替代手术组 (回盲肠间置组 )。在术后 60d、12 0d进行口服葡萄糖耐受实验和核素胃排空实验 ,且观察术后动物恢复情况。结果 术后 2个月 ,B I式组和回盲肠间置组与对照组比较 ,相差均非常显著 (P <0 0 1) ;该两组对比 ,无显著差异。 4个月后 ,回盲肠间置组胃排空时间延长 ,几乎接近正常对照组 ,与B I式组比相差非常显著 (P <0 0 1)。结论 回盲括约肌能替代幽门部分功能 ,回盲肠间置替代幽门重建消化道具有可行性。
Objective To evaluate the application of ileocecum interposition (Ⅱ) graft as pylorus replacement in alimentary reconstruction. Methods Twenty-one minipigs were randomized into three groups: sham operation group (controls), B-I group, and II group. Gastric emptying was quantified using oral glucose tolerance test (OGTT) and radioscintigraphy at 2 and 4 months after operation. Postoperative recovery of animals was observed. Results At 2 months after operation, no significant difference was found between the II group and the B-I group, while their gastric emptying time was significantly faster as compared with that of the controls (P<0.01). At 4 months after operation, animals in the II group had a significantly prolonged gastric emptying time as compared with those in the B-I group (P<0.01). Conclusion Ileocecal valve can partially substitute partly the missing pyloric sphincter. The ileocecum interposition graft can offer specific advantages over current reconstruction procedures.
