摘要
目的 测量前列腺癌 (PCA)以及良性前列腺增生 (BPH)患者中组织多肽特异抗原 (TPS)血清的表达水平 ,并与前列腺特异性抗原 (PSA)包括总PSA(T- PSA)和游离PSA(F- PSA)的表达水平进行对比研究 ,探讨TPS对PCA的临床诊断价值。方法 BPH患者 4 3例 ,经穿刺活检或手术确诊为PCA 6 1例。采用酶联免疫吸附剂试验 (ELISA法 )测定TPS ,采用时间分辨荧光方法测定T -PSA和F -PSA。测量数据应用SPSS 9.0统计软件进行统计分析。结果 PCA和BPH患者的血清TPS均有不同程度的异常升高 ,且PCA患者的TPS异常表达尤为显著 ,两者差异有显著性 (P <0 .0 1)。TPS诊断PCA的灵敏度 (75 .4 0 % )和特异性 (81.39% )均稍高于T- PSA(6 7.2 1%和 79.0 6 % )及F- PSA(72 .13%和 6 7.4 4 % )。TPS分别与T -PSA及F -PSA联合使用 ,诊断PCA的特异性可显著增加 ,分别达到 95 .34%及 90 .6 9%。结论 TPS在大多数PCA患者的血清中呈异常表达。TPS与T -PSA、F- PSA联合检测 ,可提高PCA临床诊断的特异性。
Objective To study the measurements of serum expression levels of tissue polypeptide specific Antigen (TPS) in patients with prostate cancer (PCA) and those with benign prostate hyperplasia (BPH). The comparison with the expression levels of PSA (T-PSA and F-PSA included) would lead to the exploring of the diagnostic value of TPS in PCA.Methods Of all patients, 43 were BPH and the other 61 were PCA diagnosed by operation or biopsy. Serum TPS was assayed by ELISA. T-PSA and F-PSA were detected by automatic time resolution chemiluminecent assay. SPSS 9.0 was used for data analyses.Results Serum TPS levels were elevated to some degrees in both PCA and BPH patients, with obviously abnormal expression levels in PCA. The difference between PCA and BPH was significant (P<0.01). The sensitivity and specificity of TPS in PCA were 75.40% and 81.39% respectively, higher than those of T-PSA (67.21% and 79.06%) and F-PSA (72.13% and 67.44%). The diagnostic specificities were signi- ficantly higher in combination of TPS and T-PSA (95.34%) or TPS and F-PSA (90.69%).Conclusion Abnormal TPS serum expressions are found in most PCA patients. Combined detection of TPS and T-PSA together with F-PSA can increase the diagnostic specificity of PCA.
