带状疱疹患者出疹期皮肤及皮神经超声表现

摘要 目的 观察带状疱疹患者出疹期皮肤和皮神经的声像图改变 ,为早期诊断寻求影像学依据。方法 对 37例出疹期带状疱疹患者 77处病变部位进行高频超声检测 ,并与对侧对应部位比较 ,观察皮肤、皮下和皮神经的声像图特征 ,测量皮肤、皮下厚度及皮神经直径。结果 带状疱疹患者病变处皮肤、皮下明显增厚 ,皮神经明显增粗 ,健患侧比较 P<0 .0 1。92 % (34/ 37)皮肤回声减低 ,84 % (31/ 37)皮神经线性结构模糊。结论 高频超声检测出疹期带状疱疹患者皮肤和皮神经有异常声像图表现 ,为进一步研究提供了试验依据 ,为临床早期诊断、减少误诊提供了影像学依据。 Objective To examine the imaging of skin and cutaneous nerve of herpes zoster patients at eruptive stage,and find the imaging evidence for early diagnosis.Methods Seventy-seven lesions of 37 patients at eruptive stage were examined by high frequency ultrasonography,then compared with contralateral locus in every patient,investigating the characteristics of skin,subcutis and cutaneous nerve;measuring the thickness of skin,subcutis and the diameter of cutaneous nerve respectively.Results Compared with contralateral locus, the thickness of skin and subcutis obviously increased,and diameter of cutaneous nerve increased too (P<0.01). 92%(34/37) patients′ skin echo decreased;84%(31/37) patients′ cutaneous nerve echo decreased,and their linear structures were vague.Conclusion At eruptive stage of zoster,skin and cutaneous nerve had abnormal ultrasonic findings.The results provided the imaging evidence for early clinical diagnosis and reducing misdiagnosis of herpes zoster.
