肺癌不同放疗技术剂量学比较研究 被引量:3

摘要 目的 :应用三维治疗计划系统评价肺癌不同照射方法的剂量学差异 ,以明确肺癌三维适形放射治疗的优势。方法 :选用8例 期非小细胞肺癌病例 ,采用 Topslane三维治疗计划系统设计放射治疗计划 ,每例均做 3个计划 ,分别为常规、常规加适形和适形放射治疗计划 ,计划的总剂量均为 6 6 Gy。用剂量体积直方图 (DVH)比较靶区剂量和正常组织的受照射剂量的差异。结果 :3种治疗计划均能满足靶区剂量要求 ,剂量变化差异无统计学意义。常规、常规加适形和适形放射治疗计划的照射适形指数分别为 0 .1 1、0 .2 1和 0 .32 ;全肺照射≥ 2 0 Gy的体积百分比的均数分别为 (38.4 5± 6 .5 4 ) %、(31 .0 0± 6 .33) %和 (2 6 .32± 7.35 ) % ;脊髓的最大剂量分别为 4 0、4 5和 38Gy;食管照射≥ 5 0 Gy的体积百分比的均数分别为 (38.4 3± 1 4 .33) %、(33.98± 2 0 .4 3) %和 (2 2 .5 5± 1 4 .6 7) % ;心脏照射≥ 35 Gy的体积百分比的均数分别为 (2 8.5 5± 6 .99) %、(2 1 .88± 1 2 .1 1 ) %和(1 8.4 4± 1 0 .78) %。结论 :3种治疗计划均能满足靶区剂量要求 。
