摘要
目的:探讨子宫内膜癌在围绝经期的发病特点。方法:应用B型超声和宫腔镜检查结合诊断性刮宫分析了2451例子宫异常出血病例,并对围绝经期子宫内膜癌的临床和病理特点进行了分析。结果:在围绝经期,3.1%的子宫异常出血者患子宫内膜癌;0.85%的患者患非典型增生,显著高于生育期的0.1%(P<0.01)。围绝经期子宫内膜癌患者中,非腺癌的比例数为22.6%,中、低度分化的腺癌分别为29.1%和8.3%。腺癌患者平均年龄为51.8岁,26.5%发生转移;腺鳞癌平均年龄51.9岁,37.5%转移;鳞癌患者平均年龄61.8岁,75%发生转移。围绝经期有透明细胞腺癌2例,1例转移。2例未分化癌发生在绝经期,均转移。1995年后围绝经期子宫内膜癌病人比率比1994年前上升了近18个百分点。结论:子宫内膜癌是围绝经期子宫异常出血的重要原因;围绝经期子宫内膜癌发病率呈逐渐上升趋势,非腺癌的比例呈升高的倾向。
Objective In order to research the clinical characters of endometrial carcinoma in perimenopause.Method2451cases with abnormal uterine bleeding were analyzed by dilation and curettage of uterus or surgery combining with Type B ultrasonography and hysteroscopy,and the clinic and pathologic characteristics of endometrium were analyzed simultaneously.Results3.1%of peri-menopausal women with abnormal uterine bleeding suffered from endometrial carcinoma;0.85%of the cases suffered from atypical hy-perplasia,and was higher in perimenopause than0.1%in child-bearing period(P<0.05).22.6%of cases with endometrial carcinoma was non-adenocarcinoma ,and the ratios of adenocarcinoma with mid-or low-differential degree were29.1%and8.3%of adenocarci-noma cases in perimenopause,respectively.Average age of adenocarcinoma patients is51.8years old,and the peripheral nodes and tis-sue metastasis was seen in26.5%of the cases;Average age of adenosquamous carcinoma patients is51.9years old,and metastasis hap-pened in37.5%of the cases;75%of squamous carcinoma (average age61.8)had metastasis of peripheral nodes or tissue.There were2cases of clear cellular adenocarcinoma (1had metastasis)in perimenopause,and2undifferential endometrial carcinoma (all metastasis)in postmenopause.The constructive ratio of endometrial carcinoma in peritmenopause was about 18%higher since1995than that in decade from1985?1995.Conclusion Endometrial carcinoma is the main cause of abnormal uterine bleeding in perimenopausal peri-od;The incidence of endometrial carcinoma trends to gradually increasing,and trend of an ascending incidence of non-adenocarcino-ma had emerged in perimenopause.
