腹腔镜手术电刀切开胆总管对其愈合影响的动物实验研究 被引量:2

摘要 目的:以胆囊模拟胆总管,研究腹腔镜手术中电刀切开胆总管对切口愈合的影响。方法:将40只大白兔随机分为电刀组和刀片组,分别以输出功率为40W的电刀和尖刀片于胆囊体部做一切口,再用3鄄0可吸收线作间断缝合。术后3、5、7、14d,观察切口愈合情况。切口组织的炎症反应及肉芽组织生长情况。结果:电刀切开胆总管切口愈合良好,无感染和切口裂开。与刀片组相比,电刀组术后3、5d时,切口处巨噬及单核细胞出现晚;术后3、5、7天肉芽组织生长缓慢;但术后14d时两组切口处炎症细胞及肉芽组织生长无差别。结论:用输出功率为40W的电刀,对胆总管切口的愈合影响较小。 Objective To study the impact of electrocision of the gallbladder mimicking that of the common bile duct during laparoscopic surgery. Methods Forty rabbits were divided into 2 groups at random, the electrocision group and the incision group. An incision was made in the body of the gallbladder by electrocision(40W) and incision respectively. The incision was closed with 3-0 absorbable sutures.On 3 d, 5 d, 7 d and 14 d after operation the incision was observed macroscopically to determine the healing effect; the inflammatory response was observed by microscopy. Results The healing after eletrocision was excellent,without infection and effraction. Compared with the incision by scalpel, the plasmocytes and macrophages of the electrcision knife group appeared late on 3d , 5 d after operation, and the granulation tisssues grew slowly on 3 d, 5 d, 7 d. On 14 d after operation, there was no significant difference between the 2 groups. Conclusions There is little influence on the healing of the common bile duct after electrocision when the electric power is maintained at 40W.
