磁刺激运动诱发电位在多发性周围神经病中的诊断价值

摘要 对12例多发性周围神经病患者进行磁刺激运动诱发电位(MEP)测定,同时测定感觉神经传导速度(SCV)、运动神经传导速度(MCV)及F波。结果发现,MEP潜伏期异常率为75%,高于SCV(66.7%)和MCV(58.2%)及F波(60%);至少有1项MEP指标异常者11例,占91.7%。分析认为,MEP异常率较高的原因在于它可以对包括神经根在内的周围神经全长进行测定。将MEP各项指标综合分析可提高MEP的阳性率。MEP对周围神经病是一项有较大价值的辅助检查手段。 Magnetic motor evoked potential(MEP)was performed in 12 cases of polyneuropathy,which including 5 cases of Guillain-Barre syndrome,3 cases of inherited sensory motor neuropathy(Charcot-Marie-Tooth syndrome,CMTS),2 cases of diabetic polyneuropathy,1 case of alcoholistic polyneuropathy and 1 case of arsenic peripheral neuropathy.At the mean time,sensory nerve conduction velocity(SCV),motor nerve condution velocity(MCV)and F-wave were done,MEP abnormality includes:1)prolonged MEP latency: 2)decreased MEP amplitude:3)conduction block: 4)dispersion.We found that 75%of our cases had prolonged MEP latency ,which is more often than the reduction and prolongation of SCV(66.7%),MCV(58.2%)and F-wave(60%).Eleven cases(91.7%)showed one or moreMEP abnormalities. A possible reason for the high MEP abnormality rate is that MEP can examine the whole path of the nerves including the distal and proximal part of the nerves.We conclude that MEP is a very useful technique in diagnosis for polyneuropathy.
