摘要
目的 :比较和评价两种方法治疗腰椎间盘突出症的的疗效。方法 :10 0例腰椎间盘突出症(LDH)患者 ,根据治疗方法不同分为两组 ,每组 5 0例。组Ⅰ采用椎间盘溶解术 (DL)治疗 ,组Ⅱ采用经皮椎间盘切吸术 (APLD)治疗。比较两组治疗效果。结果 :两组患者年龄及治疗前症状与体征无统计学差异。治疗后 1周组Ⅰ、组II的疗效有效率分别为 82 %、90 % ,优良率分别为 6 8%、80 % ,术后随访 ,于 3个月、6个月和 12个月时的优良率组Ⅰ分别为 76 %、88%和 90 % ,组II为90 %、90 %和 92 % ,两组疗效于治疗后 1周和 3个月时优良率存在显著性差异 (P <0 .0 5 ) ,6个月后优良率对比无显著性差异 (P >0 .0 5 )。结论 :APLD和DL均为治疗LDH的有效疗法。APLD的近期疗效明显优于DL ,但远期疗效两组并无显著性差异。
Objective: To compare the effects of two different therapies on lumbar disc herniation. Methods: 100 patients with lumbar disc herniation were randomly assingned to two groups: group of discolys (DL) with collagenase (groupⅠ, n=50) and group of automated percutaneous lumbar discectomy (APLD) (groupⅡ, n=50). The effects of two groups were evaluated. Results: There were no significant differences between two groups in symptoms, signs and ages before treatment. The effective rates of groupⅠand groupⅡwere 82% and 90% respectively and the good rates were 68% and 80% respectively in one week after treatment. After three, six and twelve months the good rates were respectively 76%, 88% and 90% in groupⅠand 90%, 90% and 92% in groupⅡ. There were significant differences ( P <0.05) in the good rates between groupⅠand groupⅡ in one week and three months after treatment. After six months and twelve months there were no significant differences in the good rates between the two groups. Conclusion: The therapeutic effect of APLD were higher than that of DL in one week and three months, althongh there were no significant differences between the two groups after 6 months.
