摘要
目的 检测t(11;18)所致API2 MALT1融合转录本的发生率 ,探讨其对胃MALT淋巴瘤 (extranodalmarginalzoneB celllymphomaofmucosa associatedlymphoidtissue,MALTlymphoma)病理诊断的意义。方法 收集原发性胃MALT淋巴瘤手术切除、石蜡包埋标本 2 6例为实验组 ;含MALT成分的胃弥漫性大B细胞淋巴瘤 (diffuselargeB celllymphoma ,DLBCL)和单纯的胃DLBCL手术切除、石蜡包埋标本各 10例及慢性胃炎活检石蜡包埋标本 10例作为对照组。采用半嵌套式PCR检测B细胞IgH基因重排 ;采用RT PCR检测API2 MALT1融合转录本 ,并通过形态学、免疫组化及分子生物学方法的比较 ,研究API2 MALT1融合转录本的检测在MALT淋巴瘤诊断中的意义。结果 IgH基因重排的阳性率在胃MALT淋巴瘤中为76 9% (2 0 / 2 6 ) ,在含MALT成分的胃DLBCL中为 10 0 % (10 / 10 ) ,在胃DLBCL中为 80 % (8/ 10 ) ,在慢性胃炎中为 0 (0 / 10 ) ;API2 MALT1融合转录本在胃MALT淋巴瘤中的检出率为 19 2 % (5 / 2 6 ) ,在含MALT成分的胃DLBCL、胃DLBCL和胃炎中均未检出。结论 API2 MALT1是胃MALT淋巴瘤特异性的诊断指标 ,但其阳性率相对较低 ,通过与形态学、免疫组化及B细胞IgH基因重排检测的结合 ,对胃MALT淋巴瘤的诊断和鉴别诊断具有重要价值。
Purpose To detect the incidence of API2-MALT1 fusion transcript caused by t(11;18) in gastric MALT lymphoma and to discuss its significance in pathological diagnosis. Methods Formalin-fixed and paraffin-embedded samples of 26 cases of primary gastric MALT lymphomas were subjected to semi-nested PCR to detect the B-cell IgH gene rearrangement as well as RT-PCR to detect the API2-MALT1 fusion transcript. Ten cases each of gastric DLBCLs with MALT component, pure gastric DLBCLs and chronic gastritis were served as the negative controls. The diagnostic value of API2-MALT1 fusion transcript in gastric MALT lymphoma was investigated through the comparison of morphology, immunohistochemistry, PCR and RT-PCR results. Results B-cell IgH rearrangement was detected in 20 of 26 (76.9%) gastric MALT lymphomas, while 10 in 10 (100%) gastric DLBCLs with MALT component, 8 in 10 (80%) gastric DLBCLs and 0 in 10 gastritis were positive. API2-MALT1 fusion transcript was detected in 5 of 26 (19.2%) gastric MALT lymphomas. None of the DLBCLs with MALT component (0/10), DLBCLs (0/10) or gastritis (0/10) was found positive for API2-MALT1 fusion transcript. Conclusions The detection of API2-MALT1 fusion transcript has a high specificity to MALT lymphoma. Since it has a relatively low incidence, it should be best applied in combination with morphology, IHC and B-cell IgH gene rearrangement detection in diagnosis and differential diagnosis.
