支气管肺泡灌洗液腺苷脱氨酶对肺结核鉴别诊断的临床价值 被引量:10

摘要 目的 探讨支气管肺泡灌洗液 (BALF)中腺苷脱氨酶 (ADA)浓度对肺结核的诊断和鉴别诊断价值。方法  12 2例诊断不明肺疾病患者 ,在行纤维支气管镜检查时留取BALF ,用改良的Martine方法测定其中的ADA浓度。12 2例患者根据最终确诊结果分为二组 :结核组 4 9例 ,非结核组 73例。结果 结核组BALF中ADA平均浓度为 (2 1±4 )U/L ,非结核组为 (6± 3)U/L ,前者明显高于后者 ,二者差异有统计学意义 (P <0 .0 0 1)。以BALF中ADA浓度≥ 12 .0U/L为诊断肺结核的标准 ,其诊断的灵敏度和特异度分别为 89.8%和 93.2 %。结论 检测BALF中ADA浓度对肺结核具有很高的诊断和鉴别诊断价值。 Objective To investigate the value of adenosine deam inase(ADA) in brocho-alveolar lavage fluid(BALF) for diagnosis and differential diagnosis of pulmonary tuberculosis. Methods Fibrebronchoscopies and brocho-alveolar lavage were made on 122 no-confirmed diagnostic patients with pulmonary diseases, the concentrations of ADA in BALF were assayed using improved Martine method. According to the final-confirmed diagnosis, 122 patients were classified into 2 groups: tuberculosis group, 49 cases; non-tuberculosis group, 73 cases. Results Tuberculosis group had the mean concentration of ADA in BALF (21±4)U/L, non-tuberculosis group (6±3)U/L. The former was much higher than the later(P< 0.001). 12.0 U/L of ADA concentration was taken for the criteria of diagnosing pulmonary tuberculosis, the sensitivity and specificity were respectively 89.9% and 93.2%. Conclusion The concentration of ADA in BALF has a high value in diagnosis and differential diagnosis of pulmonary tuberculosis.
