Value of ^(18)F-FDG coincidence SPECT in diagnosis of recurrent bone and soft-tissue tumors

摘要 目的 评价18F 脱氧葡萄糖 (FDG)符合线路显像在骨及软组织肿瘤术后局部复发和远处转移诊断中的价值。方法  18例骨及软组织恶性肿瘤术后可疑复发患者 ,进行 3 2例次18F FDG符合线路显像 ,显像结果与手术病理 (15例次 )或临床随访 (17例次 )结果进行对照。结果 18F FDG符合线路显像共检出 2 0例次局部复发中的 19例次 ,灵敏度为 95 % ,其中 13例次接受了针对局部复发和远处转移灶的手术治疗 ;无复发或转移的 12例次18F FDG符合线路显像中 ,阴性 10例次 ,特异性为 83 % ;阳性预测值、阴性预测值、准确率分别为 90 %、91%和 91%。结论 18F Objective To assess the value of 18 F-FDG coincidence SPECT in the detection of local recurrences and distant metastasis of bone and soft tissue tumors. Methods Thirty-two 18 F-FDG coincidence SPECT studies from 18 patients with clinically suspected of local recurrence and/or distant metastasis of bone and soft-tissue tumors were performed. The results were correlated with histopathology findings on surgical specimens (in 15 studies) or clinical follow-up data (in 17 studies). Results On a study-based analysis, 18 F-FDG coincidence SPECT had a sensitivity of 95% (19/20), a specificity of 83% (10/12) and an accuracy of 91% (29/32). The positive predictive value and negative predictive value were 90% (19/21) and 91% (10/11), respectively. Thirteen recurrent foci detected by 18 F-FDG coincidence SPECT received surgical management. Conclusion 18 F-FDG coincidence SPECT is high sensitive and specific to detect local and distant recurrences of bone and soft-tissue tumors.
