组织追踪直条图显像分析心肌节段运动技术的研制与临床应用 被引量:5

摘要 目的 利用现有的普通彩色多普勒超声仪器 ,开发研制组织追踪直条图显像技术 ,用于局部心肌节段运动的定量分析。方法 应用自制组织追踪直条图显像技术 ,随机选取心肌梗死患者 2 3例、冠心病患者 3 6例、扩张型心肌病患者 41例、高血压患者 41例 ,测定其胸骨旁短轴观乳头肌水平切面图、心尖四腔观及心尖两腔观各个节段心肌的运动速率。二维超声测左室射血分数。结果 冠心病组的组织追踪直条图所测节段运动减低与冠状动脉造影显示狭窄部位符合率较好 ,但对右冠状动脉供血区局部心肌的异常节段运动检出率偏低。心肌梗死、冠心病、扩张型心肌病组各个超声切面所测局部心肌运动速率均较对照组显著下降 (P <0 .0 0 1)。心肌梗死、冠心病、扩张型心肌病各组心尖四腔观测定的心肌节段收缩运动速率均较对照组显著下降 (P <0 .0 0 1)。采用组织追踪直条图显像技术测定二尖瓣环沿心脏长轴的运动速率与左心室射血分数呈直线相关关系 (r =0 .63 )。结论 组织追踪直条图显像技术能够用于临床定量分析心肌节段运动 。 Objective To develop a tissue tracking method with bars which allows quantification of regional myocardial tissue motion,and to evaluate the potentiality of its clinical application.Methods Doppler tissue imaging of myocardium in patients with myocardial infarction(n=23), coronary artery disease(n=36), myocardiopathy(n=41) and hypertension(n=41) were collected and regional myocardial velocity at different scanning views was measured by bars of tissue tracking imaging.Results The result of the tissue tracking method of bars in recognizing abnormal segments was well matched with the those of coronary angiography.However,the predicting rate by the tracking method was relatively low in patients with right coronary artery disease.The regional myocardial velocities decreased significantly in the patient groups(P< 0.001 ).The regional myocardial velocities in four chamber view were decreased significantly in groups of myocardial infarction,coronary artery disease and myocardiopathy(P< 0.001 ).There was good correlation between parameters of long axis motion of mitral ring and ejection fraction(r= 0.63 ).Conclusions Quantitative noninvasive evaluation of regional myocardial motion can be obtained accurately and rapidly by this modality of tissue tracking with bars.
