摘要
为观察充血性心力衰竭 (CHF)患者甲状腺激素改变以及生脉注射液对其甲状腺激素的影响 ,将 6 7例患者随机分为常规治疗组和生脉注射液组 ,两组疗程均为 2周 ,观察治疗前后三碘甲状腺原氨酸 (T3 )、甲状腺素(T4)、反三碘甲状腺原氨酸 (γT3 )、促甲状腺素 (TSH)以及左室射血分数、左室短轴缩短率、每搏出量、心输出量及心脏指数等指标变化情况 ,并以 2 2例健康者作为对照。结果 :与健康对照组比较 ,充血性心力衰竭患者T3 、T4明显降低 (P <0 0 1) ,γT3 升高 ,且T3 、T4与心功能分级呈负相关 ,γT3 与心功能分级呈正相关 ;经治疗 2周后 ,生脉注射液组总有效率显著高于常规治疗组 (P <0 0 1) ;治疗后两组患者T3 、T4较治疗前显著升高 ,γT3 显著降低 (P <0 0 5 ,P <0 0 1) ,生脉注射液组优于常规治疗组 (P <0 0 5 ,P <0 0 1)。提示CHF患者存在甲状腺激素分泌异常 ;甲状腺激素异常程度与心脏病变程度相关 ;
To observe the change of thyroid hormone in patients with congestive heart failure (CHF) and intervention by “Shengmai Injection”, 67 cases were randomized into two groups: control group which was given conventional treatment (digitalis+diuretics+ACEI) and treatment group which was given “Shengmai Injection” besides conventional treatment. Before and 2 weeks after treatment, the plasma levels of T 3, T 4, rT 3, TSH, LVEF, FS, SV, CO and CI were determined. Another 22 healthy persons were considered as normal group. Results: Compared to normal group, T 3 and T 4 in patient with CHF increased significantly, rT 3 decreased significantly (P<0.01); the plasma level of T 3 and T 4 correlated negatively to heart function grades, rT 3 positively; the total effective rate was higher in treatment group than in control group (P<0.01); the plasma levels of T 3 and T 4 increased and rT 3 decreased significantly in two groups 2 weeks after treatment, with significant differences(P<0.05-0.01); T 3 and T 4 were higher in treatment group than in control group, and rT 3 lower than control group after treatment(P<0.05-0.01). It is concluded that the patients with CHF are complicated with sick syndrome of normal thyroid function and present abnormal thyroid hormone; the abnormal degree of thyroid hormone is correlated to the grade of heart function; “Shengmai Injection” can improve heart function and rectify abnormal thyroid hormone.
