摘要 目的 探讨多排螺旋CT血管成像技术 (MDCTA)显示脊髓Adamkiewicz动脉的最佳扫描参数、强化延时时间及图像重建方法。方法  12 5例胸、腹部无明显器质性病变的连续性受检者 ,采用不同的扫描触发CT阈值、延时扫描时间和各种图像后处理方法 ,显示脊髓Adamkiewicz动脉解剖细节。结果 最佳扫描参数为扫描速度 0 .5s、层厚 0 .75mm、螺距 1.5 ;扫描触发CT阈值 170Hu ;达到该阈值后再延迟 18s后自动启动扫描 ;原始数据按 1mm层厚、0 .75mm层距、B2 0f算法进行横断面图像重建 ;MPR、CPR和MIP在显示脊髓Adamkiewicz动脉方面具有优势。结论 采用优化的扫描参数和恰当的图像重建技术 ,MDCTA能清晰显示脊髓Adamkiewicz动脉的起止关系、分支情况和走行特点 。 Objective To visualize the artery of Adamkiewicz by multi-detector-row spiral CT angiography (MDCTA), and to investigate the optimized scanning parameters, scan delay time and image post-processing methods. Methods A total of 125 consecutive normal subjects without any history of thoracic and abdominal lesions who underwent contrast-enhanced CT examination were enrolled to prospectively investigate the optimization of MDCTA techniques, in terms of scan-triggering CT threshold, scan delay time and various image post-processing methodology, for the depiction of the detailed anatomy of the artery of Adamkiewicz. Results The best scanning parameters were as follows: 0.5 s per rotation, 0.75 mm slice thickness, 1.5 pitch. CT value of 170 Hu was selected as the threshold to trigger data acquisition. When this CT threshold value was reached, the scanning was delayed for another 18 s, and then helical scanning automatically began. Axial images were reconstructed with B20f algorithm, 1 mm slice thickness and 0.75 mm interval. Multiplanar reformation (MPR), curved planar reformation (CPR) and maximum intensity projection (MIP) were advantageous in depicting the artery of Adamkiewicz. Conclusion Using the optimized scanning and reconstruction techniques, 16-slice MDCTA can clearly depict the detailed anatomy of the artery of Adamkiewicz (including the origin, the branching patterns and the entire course). As a noninvasive imaging technique, optimized MDCTA should be the method of choice for the depiction of the artery of Adamkiewicz.
