摘要
目的 介绍一种治疗Hangman骨折的新的手术方法。方法 自2001年10月~2003年6月,应用Axis系统经C_2C_3椎弓根固定术选择性治疗Hangman骨折9例。男8例,女1例,年龄21~56岁,平均38.5岁。致伤原因:车祸伤6例,跌落伤3例。根据Levine和Edwards的X线分型标准,Ⅱ型骨折5例,ⅡA型骨折2例,Ⅲ型2例。结果随访6~26个月,平均16个月。无内固定松动,骨折全部获得愈合,无明显的颈椎功能受限。结论 经C_2、C_3椎弓根固定治疗不稳定性Hangman骨折是一种有效的方法,可达到良好的固定和早期康复的目的。
Objective To introduce a new method of surgical management of Hangman's fractures. Methods From Oc-
tober 2001 to June 2003, 9 cases with unstable Hangman's fractures were treated by using Axis pedicle screw system through
C_2,C_3, including 8 males and 1 female aged for 21 to 56 years old (mean 38.5 years old). According to the classification
system designed by Levine and Edwards depending on the radiological manifestations of Hangman's fractures, they were sub-
divided into type Ⅱin 5 cases, type ⅡA in 2 cases and type Ⅲ in 2 cases. The causes of injury were traffic accidents in 6
cases, falling down in 3 cases. Results After follow-up for 6 to 26 months(mean 16 months), all the patients had no screw
loosened and the fractures were cured. The function of cervical vertebra was not limited obviously. Conclusion C_2,C_3 pedi-
cle screw fixation for selected management of Hangman's fractures is a effective method, which has proved its value as a meth-
od achieving solid bony fusion with a low rate of complications. A major benefit is to facilitate nursing care and earlier rehabili-
tation.
