摘要
目的 通过彩超动态观察 ,分析移植肾血管阻力指数增高的临床原因。方法 对 1 35例肾移植患者从术后 1 5 h～ 2年行彩超动态观察。结果 1 35例肾移植患者中 ,移植肾血管阻力指数增高 (RI>0 .80 )者 36例。其中超急性排斥 1例 ,急性排斥 1 5例、急性肾小管坏死 4例、环孢素中毒 2例、肾周积液 1 0例、肾积水 4例。结论 移植肾阻力指数增高是一种异常表现 ,其变化早于临床症状和形态学改变 ,但它是一种非特异性的表现。引起原因为 :1 .排斥反应 ;2 .急性肾小管坏死 ;3.环孢素中毒 ;4 .肾外压迫 ;5 .
Objective To analyse the clinical causes of the elevation of resistance index (RI) in transplanted kidney by continuously ultrasonographic monitoring.Methods Successive 135 renal transplant patients were observed from 15 hours to 2 years after operation.Results Of the 135 renal transplant patients there were 36 patients with high RI>0 80 including:super acute rejection in 1 case;acute rejection in 15 cases;acute tubular necrosis in 4 cases;Cyclosporin A(CSA) toxicity in 2 cases;peritransplant fluid collection in 10 cases;hydronephrosis in 4 cases.Conclusions The RI elevation of transplanted kidney is always an abnormal phenomenum and the changes are earlier than the clinical manifestations and morphology changes,the causes of which are as follows:(1)rejected reaction;(2)acute tubular necrosis;(3)CSA toxicity;(4)extrarenal compression;(5)urethral obstruction.
