摘要
目的 探讨取栓加溶栓联合治疗下肢深静脉血栓形成的疗效。 方法 回顾性分析1996年 11月～ 2 0 0 3年 2月收治的下肢深静脉血栓形成 2 8例 ,采用手术取栓 ,溶栓、抗凝、祛聚、支持等综合措施治疗。 结果 本组手术均成功 ,围手术期无死亡 ,无出血、肺栓塞等并发症。术后 2周行患肢深静脉顺行造影 ,6例 ( 2 1.4 % )主干深静脉完全通畅 ,其余 2 2例不完全通畅 ,5例大隐静脉 -股静脉吻合口通畅。 2 3例 ( 82 .1% )临床症状消失。 2 5例获得随访 (占 89.3% ) ,平均随访时间 18.6 ( 4～ 2 8)个月 ,发生深静脉血栓形成后遗症 5例 ( 17.9% )。 结论 取栓加溶栓联合治疗下肢深静脉血栓形成近期效果满意 ,可明显降低下肢深静脉血栓形成后遗症的发病率。
Objective To value the effect of surgical thrombectomy combined with thrombolytic therapy for deep venous thrombosis of lower extremities. Method 28 patients with deep venous thrombosis of lower extremities admitted from November 1996 to February 2003 were retrospectively analyzed. The combined managements were surgical thrombectomy, anticoagulation or antiagglutination therapy, and maintenance therapy. Results All the surgical operations succeeded without perioperative complication such as hemorrhage or pulmonary embolism. No patient died. All the patients received ascending venography after 2 weeks and showed 6 blood stream completely smooth, 22 uncompletely smooth, 5 great saphenous femoral vein shunt smooth. The clinical symptoms dissappered in 23 cases. 25 patients were followed up for 4～28 months (mean 18.6 months) and 5 cases left post thrombotic syndrome. Conclusions The integrated therapy can decrease the morbidity of deep venous thrombosis sequal with satisfied short term effect.
