摘要
目的 通过患者的预后来探讨乙状结肠法人工阴道成形术治疗先天性无阴道经血潴留的疗效。方法 对我院收治的 1 7例接受了乙状结肠法人工阴道成形术的先天性无阴道经血潴留患者 ,进行术式分析及术后状况随访。结果 1 7例患者婚后均获得了较满意的性生活。未引起肠痉挛性腹痛 ,阴道宽度可容纳二横指余 ,深度在 1 0cm以上 ,粘膜柔软有弹性 ,并有一定肠液分泌使粘膜滑润。 1 7例患者术后均未放置阴道模具 ,无扩张阴道的痛苦 ,也无阴道狭窄发生。 1 7例中 1 0例因宫颈发育不全而行子宫切除 ,7例保留子宫 ,术后均有正常月经来潮 ,经血排出通畅 ,未再发生经血潴留 ,无痛经症状。这其中 2例于婚后 1年妊娠 ,其 1例妊娠经过顺利 ,于孕 38周胎膜早破而行剖宫产娩出一男活婴 ,婴儿发育正常未见畸形。另 1例自然不全流产 ,在当地行清宫术。结论 乙状结肠代阴道治疗先天性无阴道经血潴留其人工阴道具有与正常人阴道几乎相同的生理功能。
Objective To discuss the prognosis of retained menstruation due to congenital absence of vagina treated by reconstruction of vagina with sigmoid. Methods Seventeen women with retained menstruation due to congenital absence of vagina were treated by reconstruction of vagina with sigmoid in our hospital and then were followed up. The operative method and prognosis were analyzed. Results All of the 17 patients were satisfied with their sexual activity postoperatively, without spasmic abdominal pain. The artificial vagina was more than 2-finger's in width, more than 10cm in depth, and the mucosa was soft, elastic, and lubricated by the fluid secreted by sigmoid. After operation, no vaginal stenosis occurred and no modeling was needed, which may cause pain. Ten of them underwent hysterectomy due to aplastic cervix. For the other 7 women, uteri were maintained and regular menstruations were achieved without retained menstruation or dysmenorrhea. Two out of seven women got pregnancy after 1 year of marriage, and one of them obtained a well-developed boy by cesarean section at the 38th gestational weeks. The other one suffered incomplete spontaneous abortion and received curettage. Conclusions The artificial vagina from sigmoid can obtain the normal physiological function. The reproductive function should be maintained for those patients with normally developed uterine cervix.
