摘要
目的 观察单价抗眼镜蛇毒血清溶液滴眼治疗中华眼镜蛇毒 ( Naja naja atra,Chinesecobra)不慎进入眼睛引起外伤中毒性急性角膜炎的临床效果。 方法 观察 1 992～ 2 0 0 2年我院急诊救治眼镜蛇喷毒或加工蛇毒时不慎蛇毒进入眼睛引起外伤性急性角膜炎 8例男性病人 ,从受伤到就诊时间最快 1 5 min,最慢 5 0 min。就诊后立即用生理盐水冲洗受伤眼睛 ,紧接着给予单价抗眼镜蛇毒血清溶液滴入患眼 0 .5 h后 ,用氯霉素眼药水、可的松 (或地塞米松 )眼药水交替滴眼 ,直至痊愈。 结果 使用抗蛇毒血清滴眼后局部疼痛、异物感等症状在 2 0 min内得到缓解 ,3天内眼睑红肿、结膜充血等角膜炎症状消失。 8例病人均治愈 ,未留有后遗症。 结论 :单价抗眼镜蛇毒血清滴眼治疗中华眼镜蛇毒致眼外伤是非常方便有效的方法 ,可能与其能迅速中和眼睛内残留的蛇毒有关。
Objective In order to observe the clinic effects on mono antivenom eye-drop for treatment of ocular injury to be poisoned by the venom of Chinese cobra Naja naja atra. Methods Study on 8 cases of ocular injury by the cobra venom from 1992~2002 in emergency department. All patients' eyes had been washed with 0.9% NaCl liquor upon arrival in emergency room before antivenom eye-drop was used. After 30minutes treatment with antivenom, the chloromycitin and cortisone (or dexamethason) eye-drops were used by turns until recovery. Results After treatment with antivenom eye-drop, all patients felt the local pain and eye winker were relieved within 20min, acute keratitis disappeared within 3 days,leaving no after effect. Conclusion: Antivenom eye-drop is a convenient method and has good effect on treatment of poisoned eye trauma by cobra venom.Maybe the antivenom can rapidly counteract the remnant venom in the eye.
