摘要
目的 :探究电刺激运动诱发电位 (MEP)对脊髓病变的定位诊断价值。方法 :经颅对皮层运动区及脊髓行电刺激多导联描记肢体肌的MEP ,对临床拟诊脊髓病变和非典型、非单纯周围神经损害的 39例患者 ,测定皮层刺激MEP的潜伏期 (Lcor)、脊髓刺激MEP的潜伏期 (Lsp)、中枢传导时间 (CMCT) ,并与正常对照组对比。结果 :观察组Lcor、CMCT明显延长 (P <0 .0 0 1) ;Lsp无明显差异 (P >0 .0 5 ) ;39例中 34例分别进行了CT、MRI、椎管造影等相关检查 ,其中 30例 (88.2 4 % )在MEP提示的相应节段发现有压迫脊髓的病灶。结论 :MEP对脊髓病变尤其波及运动系统时是一种非常有用的、无创、简便而准确的电生理学定位检查方法。
Objective:To study the value of the level diagnosis of motor evoked potentials(MEP) produced by means of the electrostimulation for the wrong in the spinal cord.Methods: The 39 cases were clinically diagnosed as wrong in the spinal cord and no-typical,no-simplex peripheroneural wrong.The MEP by multi-channal record were selected by means of electrostimulation in cerebral cortex and spinal cord by way of skin. The items observed were as followed: latency of MEP produced by cortex stimulation (Lcor),latency of MEP produced by spinal cord stimulation (Lsp) and central motor conduction time (CMCT),then contrasted with the normal values. Results: The Lcor and CMCT in observation group had marked prolongation (P<0.01),but the Lsp were not markedly different between observation and control groups ((P>0.05)). (34/39) patients were respectively inspected by means of CT,MRI and /or spinogram. In 34 patients inspected, the focuses constricting the spinal cord were showed at the sites pointed up by MEP in 30 patients (88.24%). Conclusions: At the appearance of the wrong in the spinal cord,especially involving the kinetic system, the MEP are much beneficial, no-traumatic, simple,convenient and aceurate methods for the location of the electroneurophysiological detection.
